ENGLISH (EN) DISTRICT GRANT 2021-22 : APPLICATION FORM Application should be sent to Grant Chair PP Sriyani Ranatunga with all the supporting documents by Email [email protected] or by registered post on or before 30th September 2021. Postal Address: PP Sriyani Ranatunga, No. 36/50, Rosmead Place, Colombo -07. All Rotary clubs are requested to provide the following information related to the district grant application. Step 1: Basic information Name of the Rotary Club ROTARY CLUB OF COLOMBO WEST What’s the name of your project? Planting AndBeautification of Kannatta Cemetry Borella, What type of project are you planning to implement? Planting trees, Developing 5 Butterfly Gardens . All district grants activities should be selected within the Rotary's areas of focus. Select at least one area and it should be benefited by a community in large. (Health care facilities, Disease Prevention and Treatment, Water and Sanitation, Maternal & Child Health, Basic Education and Literacy, Peace building, environment and other humanitarian services). Step 2: Committee members Project committee should consist of primary contact and other two committee members. The primary contact will be the President of the club 2021 /22 and the secondary contact will be the President 2022/23 and the third member will be any board member 2021/22 (Foundation chair or service projects chair or nominated board member). This three-member committee will be responsible for district grant-related correspondence and reporting to the District Rotary Foundation committee. Step 3 : Project overview What are the main objectives of the project, and who will benefit from it? Main Object of the Project:Large area is available at Borella Cemetry. The benefits of urban green spaces span environmental, economic, cultural and political domains. These benefits are interrelated, with each cumulatively feeding into the creation of resilient and sustainable urban landscapes. Given the pressure on governments to plan for greater populations, increased urban density and climate change adaptation, there is a clear opportunity to communicate the importance and benefits of urban forests in creating resilient, sustainable cities that provide healthy and enjoyable places for people to live and work. educe air pollution, air-borne particulates and greenhouse gas emissions: Vegetation ameliorates air pollution and reduces greenhouse gases. Step 4: Project location Where will your project take place? Kannatta/Borella Cemetry Note: Project location cannot be changed from the original location which was requested in the application. Step 5 : Project Commencement date & project completion date dPartoejeoctvCerovmiemwencement date: Feb 2022. Project Completion Date: March 2022 ( Note: Project duration will be maximum 4 months & the final report should be submitted to the district on or before 30th April 2022) Step 6: Rotarians participation Describe the role that Rotarians of your club will have in this project. Rotarinas from Rotray club of Colombo West, Along with Federation of Enviornmental organization,Srilanka will participate in the Project in Planning,Implementation, Planting, and ongoing monitoring to see to it, the Project will sustain on logterm basis.

Step 7: Outcome of the project Describe the outcome of this project Community and Social Benefits • Reducing sun exposure: Shade alone can reduce overall exposure to UV radiation by up to 75%. Trees provide the best form of natural shade, with broad canopied trees being the most effective. • Improving mental wellbeing: by decreasing high blood pressure and stress. This, in turn, contributes to the well- being of urban communities. Step 8: Direct and Indirect benefits of the project 5 Describe the direct and indirect benefits of this project 1.Direct Benefits:Urban Trees,Better Air Quality.Reduced Heart Attacks,Strokes and Asthma.Reconnecting children with Nature. 2.Indirect Benefits:Healthier People Improved Neihbourhood Air quality. Step 9: Sustainability of the project Describe the sustainability of this after you complete this project Colombo Muncipality will take care of the Trees and Butterfly Garden.Federation of Enviornmental organization/Sri Lanka and Rotray club of Colombo West will Monitor on regular basis. Step 10: Publicity of the project Describe ways of publicity you are planning to do for this project. There will Be Signage from Rotary Club of Colombo West and FEO Srilanka at project site.Also there will be Print and Electronic media Publicity of the Project.

Step 11: Project budget What is the budget for this project? List each item in your project's budget. The project's total budget must equal its total funding, which will be calculated in the step 12. Step 12: Source of Funding The minimum total project value should be Rs. 150,000/=. The district Rotary Foundation committee has approved Rs. 125,000/= the club should match with minimum Rs.25,000/= to each club to carry out the district grant project during the Rotary year 2021/22 on or before 30th of April 2022. Total LKR: 421000.00 Step 13 : Financial Management 1. Financial Management Plan: The club must have a written financial management plan to provide consistent administration of grant funds. 2. Project account: Club account can be used for the district grant 2021/22. 3. Report on Use of Grant Funds: The club must adhere to all TRF reporting requirements. Grant reporting is a key aspect of grant management and stewardship, as it informs TRF of the grant’s progress and how funds are spent. 4. Document Retention: The club must establish and maintain appropriate record keeping systems to preserve important documents related to District Grants for a period of 5 years. Retaining these documents supports transparency in grant management and assists in the preparation for audits or financial assessments. Step 14: Authorization and Agreement This district grant document is an agreement between the Rotary club and the Rotary district 3220 and acknowledges that the club will undertake measures to ensure the proper implementation of grant activities and proper management of Rotary Foundation grants (District Grants 2021/22). By authorizing this document, the club agrees to comply with all of the conditions and requirements of the MOU and the district grants 2021/22 procedures. On behalf of the Rotary Club of Colombo West, the undersigned agree to comply with all of the conditions and requirements of the district grants 2021/22 and we will submit the final reports to the district Rotary foundation committee on or before the stipulated time given in the district grant application.
