SOP DEVELOPMENT Standard Operating Procedures as per OUT PATIENT DEPARTMENT PROCEDURE Rev # 0organizational policy AS/OPDR/DP-01 Date : May 09 Page# 1of 3 Process descriptors; reporting formatsand templates. 1.Objective: To ensure procedures for effective operations of Outpatient Quality processes. department of AL-SHIFA (AS). HIS Implementation 2.Scope: To lay down procedures for the registration and consultation process Operations Manual pertaining to the OPD of AS. 3.Responsibility : Primary responsibility : FOE,PRE, OP nurse Secondary responsibility :PRE, PE, OP nurse Reported to –Duty Manager 1.Procedure: 1.Patient/Patient attendee arrives at OPD counter enquires about the consultant for which they have come. 2.PRE shall greet and ask the patient politely the reason for visiting AL –Shifa . Ask them what the problem is and confirm if they have a pre booked appointment. 3.PRE checks whether patient is New/Follow up and then inform the patient about doctor availability, timing, treatment plan and probable cost of treatment and shall route to registration counter. 4.Front office Executive (FOE) shall check at registration counter if patient is new, review or referral. Collect referral or recommendation letter/slip for patients referred to AS from elsewhere and file it in Referrals file. 5.For review/follow up cases, FOE shall collect UHID card, check the details, update consultation details in HIS and route to billing counter. 6.When patient forgets the UHID card for follow up ask for patient details like name and contact number, trace the file in HIS and register the case. 7.For new registrations FOE shall get patient to fill registration form (Refer AS/OPD/RFF/01) and shall enter the details in the HIS, issue case sheet (Refer: AS/OPD/CSF/03) and shall generate UHID card (Refer: AS/OPD/UCF/02) and issue identity card to patient at registration counter and handover patient to PE. 1.To generate UHID card, PE shall get the patient inside the registration counter where digital photo ID creator installed. Get patients’ photo clicked and generate the UHID. 18 2.PRE shall counsel the patient about the importance of UHID card and ask them to carry every time the visit the
