Aged 83, she now lives alone in Dipti Sinha Bangalore. Recently Samarth editorial team met him to understand his expert views on present elder care eco system in India and the need for emphasis on cognitive health for better living. Samarth team: Sir what in your opinion, are the major factors affecting elder care in India? Dr. Grewal: In 2001, population of elders in India was around 7% of total Indian population. Interestingly the share of elderly in India i.e., those aged 60 years and above, has continued to rise steadily to nearly double from 8.6% in 2011 to 16% by 2041. In states like Kerala, HP & Rajasthan, it is 14-15%. There are two aspects to this astronomical figure and increase: 1. Social 2. Economical Society sees the elderly as non-productive and therefore a burden and wants to ignore them. They look after them out of pity whereas the elderly have equal rights as any other citizen in the country. This mind-set is not only prevailing in society but also the government who seems to think they are doing a favour by framing policies without consulting the elderly population. There are challenges like increasing healthcare costs, caring for elderly, etc related to ageing being faced by the mature economies today. These are the major factors leading to present state of elder care in the country. Samarth team: Is there any policy level action being taken to tackle this situation? Dr. Grewal: As a response to the situation, WHO has declared that the way out is active ageing. The special UN assembly general on 15th September 2018 talks about rights of older persons in the context of international policy documents on age. (https://www. un.org/esa/socdev/documents/reports/Ageing.pdf) The international community has established various acts and charters like: UN CHARTER - United Nations Principles for Older Persons The Principles provided that older persons in society ought to have independence, the ability to participate in society, have access to care, be entitled to self-fulfilment and the full dignity of life among other rights. WHO defines Healthy Ageing “as the process of developing and maintaining the functional ability that enables wellbeing in older age”. COVER STORY the capabilities that enable all people to be and do what they have reason to value. Samarth team: Are these charters being implemented in India? Dr. Grewal: We have adopted these charters and launched it in India focusing on longevity, as there are minimum awareness about the above-mentioned initiatives. We have worked in creating long-term care guidelines, collaborated with change agents in the eco system and creating community level awareness. Elders can now apply on a plain piece of paper to the District Magistrate to sanction Rs 10,000 to provide financial assistance through the local police Thana. Our organization empowers elders in self-care and help them prepare for long-term care. Samarth team: What are some of the areas that your organization works in? Dr. Grewal: Age friendly India focuses on issues beyond diseases; nutrition, mental health, frailty, abuse and immunisation and how these improve quality of life and dignity. They have designed vertical programmes to, 1. Partner with NGOs for implementing the same 2. Elder people who benefit from these programs and also act as champions of the cause 3. Work with South Delhi municipal ward. Samarth team: Can you highlight a few successful impacts of these initiatives? Dr. Grewal: Until now, these interventions have created age-friendly primary care at 102 wards. Our organization is creating age friendly primary and tertiary facilities. They are helping in community liasoning with state and central government. They have also developed a booklet on how good the elderly are doing focusing on the above-mentioned five issues. This is the first of its kind self-assessment proforma designed with expert help. Samarth team: Is there anything in particular that you would like to talk about? Dr. Grewal: Elders in our country are facing Ageism (discrimination on the grounds of a person’s age). The main challenge for our organization is how to enable inclusion in society to combat ageism. Lot of positive steps are being taken as we speak; for example the starting of Eldercare OPD in Escorts, special counter for elders in AIIMS due to which there has been a 40% jump in elders using the hospital services, companies recruiting only 65 years plus experts in Mumbai leading to lowering of attrition etc. to name a few. However much more needs to be done. In Britain, there is a special Ministry for combating loneliness. In India, 17 ministries look after elder care or part of it.
