JAN 2022 | VOL.8 PREVENT NEWSLETTER Quarterly Newsletter of the PREVENT Study Prevention of Epilepsy by reducing Neonatal Encephalopathy (PREVENT) is the world's largest study on newborns with brain injuries. Led by Professor Sudhin Thayyil at Imperial College London in collaboration with experts and institutions from around the world, we aim to prevent epilepsy caused by neonatal encephalopathy through the implementation of a pragmatic intrapartum care bundle in India. Jan 2022, Vol. 8 PREVENT Study Update! We are all now geared up to begin the implementation of our care bundle on March 1st, 2022 following the various brainstorming sessions and discussions with our collaborators and principal investigators. As part of our preparation for Phase II, we successfully organized virtual meeting from December 13th - 17th 2021 involving all the PREVENT team members from UK as well as India, to discuss the various challenges faced and update on study so far at all 3 sites. Meanwhile, our training programmes such as Basic NRP and dignity games for respectful maternity care was actively going on. We coordinated with Indian Association of Pediatrics (IAP) for organizing Basic Neonatal Resuscitation Programme (NRP) at Calicut site on December 18th, 2021 which involved hands-on training for intrapartum fetal monitoring nurses and labour room staff. However, due to existing COVID pandemic third wave situation in India, the PREVENT study had to be paused for 15 days during the month of Jan 2022, in order to ensure safety of the staff and the patients. After careful consideration, we decided to restart the study on 7th Feb 2022. Amidst the situation, the team worked from home without hampering our study outcome. 2 PREVENT Quarterly Newsletter

Jan 2022, Vol. 8 Key Achievements... An insightful discussion with lots of great scientists, clinicians and our most resourceful partners, the parents of babies!! We, the PREVENT team is excited to share that our first Virtual Symposium of 2022, \"Saving Babies Brains\", was organized on January 22nd, being led by the site investigators of IMCH Calicut in order to share the consistent work and dedicated inputs of the team till date and to brainstorm ideas and solutions to the potential challenges in the way forward. This symposium involved discussion on how to prevent birth related brain injury by using a care bundle approach and also reflected the experiences from the first phase of PREVENT study that recruited over 40,000 women and the plan for the successful care bundle implementation starting from 1st March 2022. We also demonstrated the importance of birth companion, dignity games for respectful care, labour room experiences and views of parents and other important aspects such as eLCG, intelligent auscultation, brain oriented resuscitation and therapeutic normothermia. 4 PREVENT Quarterly Newsletter

Jan 2022, Vol. 8 Key Achievements... Here we share some of the key sessions- \"dignity games\" which was played between the health care professional and mothers. This game was developed by Professor (Dame) Tina Lavender and her team, in order to promote respectful maternity care. The idea of involving the parents was innovative which will be taken forward across all the sites. The parents gave their feedback that they felt comfortable being engaged and build rapport with the health care professionals. Also they shared their opinions on respectful maternity care. \"eLCG application demonstration\"- The session involved introduction to eLCG components followed by comprehensive demonstration of eLCG application based on latest WHO Labour Care Guide. 5 PREVENT Quarterly Newsletter

Jan 2022, Vol. 8 Success story of our team member Pallavi Muraleedharan (Project Manager, Bangalore) who received the NIHR Global Health Research Short Placement Award for Research Collaboration (GHR SPARC) Round I shared her career journey and placement experiences at the webinar conducted by NIHR on the topic 'Learn more about the NIHR Global Health Research Training Awards 2022'. The webinar was aimed at discussing the eligibility and the application process of two Global Health Research (GHR) training and academic career development award competitions recently launched by the NIHR. 7 PREVENT Quarterly Newsletter

Jan 2022, Vol. 8 Spotlight Our Chief Investigator Prof Sudhin Thayyil gave an Inaugural Lecture on \"Saving Babies Brains\" on December 2nd 2021, describing his journey as a clinician scientist. Prof Paul Mathews, Head of the Department of Brain Sciences in the Faculty of Medicine of Imperial College introduced Prof Thayyil and his work thus far. In the introductory lecture, Prof Thayyil described his career from Calicut Medical College as an undergraduate, Paediatric resident at AIIMS (New Delhi), paediatric neonatal training in Leicester, New Castle, and Cambridge, and in London, and finally entering into clinical research to address some of the key challenges in preventing and treating birth related brain injury. He emphasized the importance of external fellowships in a clinical academic career, and why persistence is vital. Sir Arul Kumaran Sabaratnam (Professor Emeritus of Obstetrics & Gynaecology, St George’s University of London, and senior advisor of PREVENT study) gave a vote of thanks at the end of the lecture and lead the discussion. Jan 2022, Vol. 8 My journey with PREVENT... I started my journey with PREVENT from March 2020. I was a total Dr Pallavi G newbie in research, working in PREVENT has helped me develop and fine-tune the skills necessary to work in field of clinical research. I was OBG Clinical very fortunate to be part of PREVENT project from the beginning; I was Research Fellow able to participate in baseline OBG data collection, development and implementation of the PREVENT care bundles. I also got the PREVENT opportunity to train the research and clinical team on PREVENT care Bengaluru bundles such as electronic Labour Care Guide and Intelligent intermittent FHR auscultation which helped to develop ad upgrade my teaching skills. I deeply appreciate every member of this team for their relentless efforts for this project and for demonstrating good teamwork. From my experience, a few wonderful practices that make PREVENT a unique study are: a) Weekly team meeting to discuss the updates at each site, b) The technical discussions and cross learnings between OBG and neonatal team c) Weekly audits for data collection and review of e-LCG. I am thankful for the opportunity provided to me to work with senior scientific advisors of PREVENT – Dr. Arul Kumaran, Dr. Harshad Sanghvi and Dame Tina who were always there to help us in the development of Care bundle. I had a wonderful time working with OBG Principal Investigators at all our sites, who took time to help us in research in spite of their busy clinical schedule. I thank Dr. Savitha and Dr Sudhin for providing me an opportunity to work with PREVENT. I thank all others who worked with me in the last two years, including the clinical fellows and research nurses. Jan 2022, Vol. 8 From our New Team Member... Dr Sreya Siva Prasad I am an OBGYN, having gained my MBBS and DGO from Government Medical College, Kozhikode. During OBG Clinical Research Fellow our training period when we see mothers go through PREVENT Calicut a turbulent antepartum or intrapartum period we often wonder on its impact on the babies. With each update in research, our practices change in the hopes that the outcomes of our patients and their children improve. I believe PREVENT will be monumental in the field to set standards for intrapartum care to ascertain positive baby outcomes. When the opportunity to work under the PREVENT study presented itself, I realized that this would be instrumental in laying the foundation for a future life in research. So I am truly grateful for the platform this has given to collaborate with inquisitive and scientific minds. Jan 2022, Vol. 8 Next Quarter.. The next quarter (February - April 2022) will mark the successful commencement of the care bundle implementation phase of the PREVENT study across all sites. Our research nurses will be the key members involved in the implementation of the care bundle in labour rooms across the sites. Continuous hands-on training will be offered to the research nurses in order to enhance their competencies in care bundle interventions. Thereby, we will optimize the monitoring of women in labour across our study sites. By collaborating with global researchers and academicians with expertise in OBG and Neonatology and being led by the site investigators, we are hopeful to launch the second phase of the PREVENT study. 14 PREVENT Quarterly Newsletter
