USC TROJAN CARE EPO Don’t pay more for coverage that you won’t use Convenient assess to a full range of USC-quality health care services for an affordable cost per paycheck and low out-of-pocket costs. An excellent custom-built provider network The USC Trojan Care EPO offers access to world-class Keck Medicine of USC providers, as well as an exclusive network of other excellent, carefully selected providers. While Keck Medicine continues to expand its network throughout Southern California, some specialties are not offered by Keck in all areas. That’s why we paid special attention to building an extended network of carefully screened and selected providers, so you can find the medical care you need close to home or work. How it works You must use USC Trojan Care EPO network providers; there is no out-of-network coverage (except in an emergency). When you need care, your only out-of-pocket cost is a $20 copay for most services, and the plan pays the rest after you meet your deductible ($100 for an individual, $300 for family coverage). Your copay can be even lower. The $20 copay is reduced to $10 for primary care physician (PCP) visits or behavioral health visits for every member of your family when you designate a PCP ahead of time. Plus it’s smart to have a PCP coordinate your care. And unlike an HMO plan, under the EPO you are not required to obtain a referral from your PCP before seeing a specialist. The USC Trojan Care EPO costs less per paycheck than the Kaiser HMO or the USC PPO. And as with all the 2018 medical plan options, you can lower your cost per paycheck even more by taking action to earn the healthy choices incentives. USC PPO (Formerly called the network medical plan)Most flexibility, highest costThe USC PPO works the same as the current Network Medical Plan. It offers the mostfreedom to choose where you go for care, including non-network providers. Because ofthis broad coverage, the USC PPO costs the most each paycheck.The USC PPO has three provider coverage tiers:• Tier 1 offers convenient access to Keck Medicine of USC providers.• Tier 2 allows access to Anthem Blue Cross Prudent Buyer/BlueCross providers.• Tier 3 covers out-of-network providers.How it worksYou don’t have to choose a primary care physician (PCP); you can use any provider youwant whenever you need care. How much you pay for services depends on the tier ofthe provider you choose. Out-of-network care will always cost more.For some services (X-rays, for example), you must meet an annual deductible. Once youmeet the deductible, you and the plan share the cost (coinsurance). The amounts ofyour deductible and coinsurance also depend on the provider tier you choose. 8

USC HMO (Formerly called the Anthem HMO) More access to quality providers The new USC HMO is designed specifically for USC employees. It replaces the Anthem HMO, but the plan works essentially the same way and still includes all of the same Anthem providers. The biggest difference is that the USC HMO will have two coverage tiers. The new Tier 1 offers convenient access to a custom USC network of carefully chosen partners, while Tier 2 provides access to Anthem providers. The USC HMO does not allow you to choose Keck Medicine of USC providers, although some Keck doctors are in the Anthem network. How it works The USC HMO works like the current Anthem HMO. You must use network providers; there is no out-of-network coverage (except in an emergency). . You must choose and register with a primary care physician (PCP) who will be your first point of contact, coordinate your care and make referrals.9

Kaiser Permenente HMONo plan changes for 2018The Kaiser Permenente HMO’s health care providers exclusively serveKaiser plan members. For 2018, Kaiser HMO employee contributionsare increasing.How it worksUnder this plan, you must use Kaiser providers; there is no out-of-network coverage (except in an emergency). When you need care,you’re charged a $25 copay for most services and the plan covers therest of the cost.You must choose a Kaiser doctor as your primary care physician (PCP)to be your first point of contact, and coordinate your care and makereferrals.To learn about the Kaiser HMO, visit the Kaiser website. 10

2018 Medical Plan Costs Monthly Employee Premiums USC is now offering healthy choices incentives. These incentives are deigned to encourage you to take an active role in your health care, and result in a price deduction from your healthcare cost. For more information, click here.11

2018 Vision and Dental There are only two changes to vision benefits for 2018, but they’re important ones. You’ll be enrolled automatically for vision coverage—unless you actively opt out during open enrollment. Your monthly cost for employee-only vision coverage will be about $9 per month, so if you don’t want coverage, be sure to indicate that during enrollment. The vision plan will have two coverage tiers. The plan will still be provided through the same Vision Service Plan (VSP), but you’ll enjoy lower copays and higher allowances for frames and contact lenses when you choose the new Tier 1, USC Roski Eye Institute providers. (Roski providers are also part of the VSP network.) How it works You can use Tier 1 providers or Tier 2 VSP network providers. You can also use non-VSP providers (Tier 3 below) at a significantly reduced benefit. There are no changes to Dental for the 2018 year. For more information on the Dental Plan, click here.13
