5General Orientation DayAll new employees attend Corporate General Orientation at the Welland Hospital Site. You willhave the opportunity to learn about Niagara Health, meet others, and ask questions. Yourscheduled Orientation date can be found on the final page of your official offer letter.Time: 8:30am – 12:30pmLocation: Welland Hospital Site – Auditorium. 65 Third Street Welland. Enter the building atthe main entrance, turn right and proceed past the elevators to the first hall on your left. Theauditorium is at the end of this hallway.Parking: please park in the main entrance off of Third Street. Visitor parking is to yourimmediate right. For General Orientation day only you will have your parking ticket (obtainedfrom the parking gates on arrival) validated at no charge.Pay for Day parking is $8.00 OR Paid Staff Parking is $25 bi-weekly.Bus: Click here for Welland Transit Routes and Schedules.Contact: For any questions or to reschedule orientation please contact your RecruitmentSpecialist at ext. 44200 or your manager.Refreshments are provided during the day.Dress in comfortable professional (business casual) clothes and come scent-free (e.g. noperfume or other scents that may affect others around you as we are a scent-freeenvironment.) 6Documentation SessionEach new employee is required to arrange for a Documentation Session with Compensation andBenefits as is outlined in your offer letter. If you have not yet arranged a session, pleasecontact Compensation and Benefits at 905-378-4647 ext. 44200.Your documentation session will take place at one of the hospital sites. Your documentationsession should be completed prior to starting in your new position. Please bring the followingcompleted forms to your session:  Signed Offer Letter  Privacy and Confidentiality Acknowledgment  Code of Conduct Form  Employee Information Form  Original Criminal Record Check  Void Cheque  Social Insurance Card  Proof of Date of Birth (I.e. birth certificate, passport, driver’s license)  Any relevant degrees, diplomas, registrations, and certificates  TD1 and TD1ON forms  HOOPP Enrolment Form and HOOPP Beneficiary Form (mandatory for Full Time Permanent Hires) OR HOOPP Waiver  Manulife Beneficiary Form - Group Life & AD/D (for Full Time Permanent Hires)  Manulife Enrollment Form for Benefits (for Full Time Permanent Hires)  Previous Experience Forms (Unionized employees only)The above forms were sent in a zip drive format to your email address along with your offerletter.The Privacy and Confidentiality Acknowledgement form and Code of Conduct form can befound in the Appendix section of this package.PayrollIn your offer letter you will find information concerning your rate of pay, benefit entitlements,and union information (if applicable). Any related questions that you have can be discussed atyour scheduled documentation session with Compensation and Benefits.Pay is distributed bi-weekly through direct deposit, with the pay date being Thursday. Paystubs are sent electronically to your NH email account on pay day..

7Health ScreeningIt is a condition of your employment that your health review must be initiated within twoweeks of your start date and completed within 30 days of your initial appointment. This is toensure that you, your co-workers, and our patients are safe. Please contact your OccupationalHealth & Safety nurse as listed in your offer letter. Your health review may consist of morethan one appointment. You are required to bring the Occupational Health & SafetyDepartment New Hire Health Review that is located in the appendix of this package to yourappointment. NH Health Screening Requirements Proof of immunization / bloodwork results for: Measles, Mumps, Rubella, Chicken Pox (Varicella), Tetanus/Diptheria/Pertussis, Hepatitis B(if you do not have a vaccination record,were born after 1970 and attended a school in the Niagara Region, please contact Niagara Region Public Health Department (905-688-3762) to obtain a copy. A lab requisition can be provided if you are unable to locate your blood work/immunization records.) 8Photo IdentificationYour Identification Badge (ID) identifies you as an employee of NH.Included on your badge is your photo on both sides, your job titleon both sides, and both your first and last name on one side, and solely your first name on theother side. Your team members, coworkers, patients and their families will be able to call youby name and know the role you play at NH.Your badge is programmed to give you access into required areas for your particular role /jobfunction. By this, your badge functions as an integral component of security measures at NHand must be worn at all times while at work and in an area on your person that is easily visiblefor others to view. If you lose your ID badge you are responsible for reporting that it is missingto Security and having a new one issued. A fee is applied for lost ID cards.If you opt for Staff Parking your ID badge is programmed to allow you access and exit from anyof the NH hospital sites. Staff Parking is an option that is explained further in the next section.There are 3 possible ways to obtain an ID Badge: 1. General Orientation. Security will take your photo and issue a card by the end of the session. 2. Prior to General Orientation, you are able to bring your signed offer letter and personal Identification (e.g. Driver’s License, Passport) to site Security at the Greater Niagara General Site, St. Catharines Site, or Welland Hospital Site to have an ID badge issued. Please call and notify the Security desk prior to going in. 3. During your documentation session.Staff ParkingOnce you receive your ID badge you are able to register for Staff Parking through PreciseParkLink. Employees are required to pay for this service; deductions of $27 are made bi-weeklyfrom your pay. To register, go online to www.preciseparklink.com/myparking or call PreciseParkLink at 1-888-783-7275. You may also visit the Parking Office – located in Lot B – St.Catharines Site. You will be required to provide Precise ParkLink with the first 5 digits locatedunder your photo on your ID Badge in order to activate staff parking. Further Staff Parkinginformation can be found in the Appendix section of this package.

9Uniform PolicyAt NH, staff who are visible to our patients and visitors are required to wear a uniform. Thisprovides a positive and professional impression of the Hospital while maintaining safetystandards and preventing the spread of infection to others.
