HealthHub FAQs for employees 1) What is HealthHub? HealthHub is Health Management’s occupational health portal. If you are referred to Health Management by your employer, you will interact with HealthHub to:  View and manage appointments that are arranged for you  View the report that we develop from your referral  Report any factual inaccuracies to us about that report  Manage your consent choices throughout your referral 2) How can I access help when using HealthHub? If you require help when using HealthHub, please speak to your line manager or a member of your HR team. 3) What do I do if I forget my password? On the HealthHub login page there is a forgot password link and when you click on this you will be prompted for your HealthHub username. Once you have entered this, an email will then be sent to you containing a link which can be used to reset your password. 4) How can I change my username? It is not possible to change your username however if you forget your username please click the link on the HealthHub login page and follow the instructions provided. 5) Who can unlock my account? Your account will automatically lock for 30 minutes after 3 failed login attempts. You can unlock your account by following the “forgotten passwords” procedure described above. 6) How do I view my details? To view your account details, click on “My Details” from the menu at the top right hand side of the page. If you need to amend any of your contact details, you can select “edit details” and update/amend as appropriate.

7) How can I view information about my referral or questionnaire? To view information about your referral, follow the steps below: 1. Click on “My Referrals” towards the top right of the page 2. Click on the referral reference number of the referral you wish to see more information on This will take you to a summary page, where you will be able to see:  Your details  The details of the person who referred you to Health Management  Your appointment details  Uploaded documents, including any Occupational Health reports or certificates that we develop from your referral 8) How do I know if an appointment has been booked for me? If we book an appointment for you, you will receive a notification via email asking you to login to view your appointment time/date. You will be notified as to whether your appointment has been booked to be conducted by telephone or in person. If the appointment is to be conducted in person, you will be provided with the location for the appointment. If an email address has not been provided with your referral, you will receive a SMS message with the appointment details. 9) I have received an appointment that I am unable to attend, how do I rearrange? If you receive a telephone appointment that you are unable to attend, you are able to notify us and select an alternative appointment. Please inform your manager of changes you wish to make in the first instance, and they will also be updated of any changes through HealthHub. Firstly, click on the “My Referrals” link towards the top right of the page. Click on the referral reference number that has the appointment you wish to rearrange. The next page will provide you with a summary of your referral. On this page, there is a link called “Manage Appointment”. Clicking this link will provide you with the opportunity to inform us as to why you need to rearrange the appointment then allow you to select an alternative appointment.

If you receive a face to face appointment that you are unable to attend, please speak to your manager in the first instance. 10) How do I know what to expect from my appointment? To see more information about your Occupational Health appointment, navigate your way to your referral summary as described above. On the summary page, below your appointment details will be a link called “What to expect from your appointment”. Clicking on this link will enable you to download a guide providing further information about your appointment. 11) I’ve asked to view my Occupational Health report before my employer sees it. How do I see the report and report any factual inaccuracies? To see your Advice Report from your referral, firstly click on the “My Referrals” link located towards the top right of the page. Locate your referral on the list, and click on the referral reference number. You will then be able to see a summary of your referral. Mid-way down the page will be a link called “Consent Advice Report”. Click on this link and on the next page you will be provided with the opportunity to notify us of any factual inaccuracies you identify in the report. Please note that, depending upon your consent choice, you will have a limited time in which to do this from when we release the report to you before it is released to your employer.
