Statement of WorkIMPLEMENTATION, TRAINING AND CONVERSION SERVICESOverviewThe Agency has contracted with The Echo Group (Echo) for an Implementation of EchoVantage. The specific services to be performed by Echo during theImplementation are outlined below and this Statement of Work encompasses all services to be performed by Echo. Additional services hours may be purchased at thestandard services rate at that time.Echo and the Agency will work collaboratively to establish a detailed Project Plan for the Agency’s Implementation of EchoVantage as outlined in the Scope of Servicesbelow. The Project Plan shall dictate the Implementation Schedule and Echo and Agency resources and responsibilities. The implementation schedule, outlined in theProject Plan, shall be no more than six (6) months. Action or inaction on the part of the Agency that should extend the schedule past nine (9) months from the date ofAgreement Execution may necessitate a Scope Change Order and potentially additional costs.Travel and ExpensesTravel and expense costs are reimbursable under the terms of Exhibit H (Expense Reimbursement Policy). All services are scheduled to be completed remotely but ifonsite services are requested, travel costs will apply.Assumptions and ExpectationsExpectations of the Agency:• The Agency will assign the appropriate resources knowledgeable in Agency clinical workflows to act as Super-Users and participate in configuring the system.• The Agency will collaborate with Echo to create a comprehensive Project Plan and implementation timeline and will make a good faith effort to complete tasks as outlined in the Project Plan timeline.• The Agency will respond to requests for information required to complete system implementation promptly and thoroughly.• The Agency will provide Echo assigned resources access to the Agency’s EchoVantage SaaS-hosted database for use in configuration guidance and training.• The Agency will complete thorough system testing within the agreed upon Project Plan timeline and will be responsible for identifying and tracking the remediation of any testing issues.Expectations of Echo:• Echo will provide project team members knowledgeable in the Echo software configuration and use to provide the services outlined in this Statement of Work.• Echo will collaborate with the Agency to create a comprehensive Project Plan and implementation timeline and will make a good faith effort to complete tasks as outlined in the Project Plan timeline.• Echo will assist the Agency in the identification and remediation of any testing issues in which the system does not function as designed.• Echo will complete a Go Live Readiness Assessment with the Agency within 10 days of System Cutover to advise the Agency on Go Live Planning.

Statement of Work Scope of Services Echo will collaborate with the Agency to create a Project Plan to detail tasks, due dates and responsibility.Echo Provided ServiceImplementation Assistance Echo will conduct bi-weekly status calls with the Agency to review progress against the Project Plan. All configuration is completed by the Agency with Echo providing remote Super-user training and guidance to the Agency in the configuration of EchoVantage to support Agency workflows. Echo will provide remote support during Agency Go live of EchoVantage and assist Agency to document and remediate Go Live issues.Forms Library Echo will provide the Agency access to Echo’s Form Library of existing standard Forms. The Agency may select a total of ten (10) web-based forms for install into the Agency’s database.End-user Training Echo will provide remote End-user Training sessions (for a total of 16 hours) to the Agency to include: • User Groups and Staff Set-up • Service Components and Definitions • Registering a Client and Scheduling a Service • Adding a diagnosis • Episodes and Enrollments • Using the Visual Health Record • Entering a Service and Completing a Progress Note • Creating a Treatment PlanIf additional requirements for services are identified during the course of the implementation, a new Statement of Workand Proposal authorization may be required.
