Garfield County benefits include medical, dental, visionand prescription drug coverage for employees and de-pendents. These are available to employees working 30 ormore hours per week.Premiums for the base plan (PPO4) for the employee anddependents are paid 100% by the County. Buy-up optionsare available. Coverage begins the first of the month fol-lowing date of hire. DENTAL CARE Coverage of preventative services including routine exams and cleaning for twice a year up to the reason- able, necessary and customary charges. Additional services available with coinsurance and deductible. PRESCRIPTION DRUG COVERAGE Retail for 30 day supply: Generic $20, Preferred: $40, Non-Preferred: $60. Mail Order—for 90 day supply: $40 / $80 / $120 FLEX SPENDING Medical FSA allows employees to put away up to $2,550 pre-tax for medical-related expenses. Dependent Care FSA allows employees to pay for childcare expenses up to $5,000 per year pre-tax. LIFE INSURANCE AND VOLUNTARY LIFE This plan provides a $30,000 term life insurance and AD&D benefit for employees.. Additional life insur- ance may be purchased for employees and their de- pendents.
