OPERATIONAL PROCEDURES 20 Operational procedures form part of our wider Business Management System (BMS). Process and procedures are built around best practice, set a standard for managing risks across UOS and detail the safe way of performing tasks and activities. The benefits of procedures includes; the ability to consistently provide our services that meet our customer and applicable statutory and regulatory requirements actively facilitates continuous improvement of our employees’ morale, safety and performance having a strong, consistent standardised way of working means that UOS is more efficient across all aspects of our business

RISK ASSESSMENT 21 A risk is measured by the combination of the probability of a perceived threat or opportunity occurring and the magnitude of its impact (severity). Risk assessment is not about creating huge amounts of paperwork, but rather about identifying sensible measures to control the risks on our worksites. A risk assessment should be completed by those with a knowledge of the task, process or equipment being assessed and include others who will be involved in the task. The basic steps of risk assessment are; Continuously identify and monitor what could cause injury, illness, harm or be a threat to UOS (hazards) Consider all those who or what could be affected Decide how likely it is that someone, the environment or UOS could be harmed and evaluate how seriously (the risk) • Take action to eliminate the hazard, or if this isn’t possible, control the risk to as low as reasonable practical (control measures, risk mitigation)

PERMIT TO WORK 22 PERMIT TO WORK The purpose of the Permit to Work System (PTW) is to clearly define the safe working procedures to be implemented to control hazards such as; hot work, entry into restricted / confined space, working at height, electrical, over side, explosive, radia6on, removal of safety interlocks, etc. The Permit to Work (PTW) must define clearly; §  The work to be done, the equipment to be used and personnel involved §  Lines of authority and responsibili6es §  Risk control measures and precau6ons to be taken when performing the task §  Ac6ons to be taken in an emergency §  Other workgroups to be informed of work being performed in their area, simultaneous opera6ons §  Authorisa6on for work to commence §  Dura6on that the permit is valid §  Method to extend the permit for an addi6onal period §  Verifica6on that all work has been completed and the worksite restored to a clean, safe condi6on

TOOL BOX TALK (TBT) TOOL BOX TALK (TBT) 19 23 The Toolbox Talk (TBT) should be carried out before the start of a task or ac6vity and if necessary during and then aRer the task is finished (to discuss lessons learned). STOP THE JOB 26 STOP THE JOB 22 Stop the Job also known as Stop Work Authority You are empowered to stop the job when you believe it is necessary to do so if; §  Your safety or the safety of others is threatened §  There is the poten6al of harm to the environment §  There is the poten6al of harm to our assets §  There is the poten6al of harm to our reputa6on Supervisors / Line Managers must; §  Support any individual or team who raise concerns and use the Stop the Job tool §  Respond promptly and seriously to a Stop the Job §  Co-ordinate ac6on to assess and implement effec6ve solu6ons to allow the tasks / ac6vity to con6nue §  Acknowledge an individuals’ and / or Teams posi6ve contribu6on to safety

TIME OUT FOR SAFETY (TOFS) 27 Time Out for Safety also known as Safety Stand Down Time Out for Safety (TOFS) is a tool to be used following an accident, incident, Stop the Job or to communicate safety matters, QHSE Notices etc. The purpose and objective of the TOFS is to get individuals and teams together to facilitate safety conversation. Where necessary a record of the TOFS should be kept along with a record of the attendees. If a TOFS is held immediately following an accident / incident; what happened, why it happened and importantly what remedial actions need to be taken to allow the task / operation to continue, should be discussed, agreed and authorised.



ACCIDENT / INCIDENT REPORTING ACCIDENT / INCIDENT REPORTING 29 An accident or incident (undesired event) is defined as an unplanned event, or series of related events which uninten6onally results, or had poten6al to result in: §  Harm to people §  Environmental impact §  Asset / equipment damage When any undesired event occurs, the immediate priori6es are as follows: 1.  Make the area safe to avoid any or further harm to personnel, the environment or asset / equip damage 2.  Treatment and recovery of any injured person(s) 3.  Ini6a6on of emergency response protocol if required 4.  ‘Securing the scene’ to allow appropriate evidence to be gathered Evidence includes; witness CCTV / photos electronic hard copy tools plant / statements video documents documents equipment No6fica6on to shoreside of all accidents and incidents should take place as soon is reasonably prac6cal following an event to enable the engagement and interface with relevant interested par6es (i.e. Snr Management, Client / Charterer, Regulatory Authori6es etc.)

ROOT CAUSE ANALYSIS 30 ROOT CAUSE ANALYSIS Root Cause Analysis (RCA) is an inves6ga6on tool to be used to determine the factors which directly contributed to the event or made maZers worse, i.e. organisa6onal deficiencies, human error, system / process / equipment failure. Other factors which indirectly contributed to the event (underlying causes) will also be uncovered during the RCA and s6ll require ac6on to be taken. By going through a structured RCA process we can effec6vely learn lessons and ensure that we make improvements to the way we operate.
