1. INTRODUCTION Established in 2010, Amlotus, LLC, was founded by a group of ESL professionals with the goal of offering quality ESL programs to students. Amlotus has grown not only in numbers, but also in the scope and breadth of its programs. Today, Amlotus has matured into a comprehensive ESL and TOEFL test preparation center. Through its growth, Amlotus has not forgotten its guiding principle- to instill a passion for English language learning so that the lives of its students are enriched and a vibrant community is built. 2. MISSION STATEMENT Amlotus, LLC, is committed to providing high-quality English instruction in the form of ESL, TOEFL classes, and tutoring in Language Arts, as well EFL/ESL Teacher Training. We strive to assist students in their academic, cultural, linguistic and professional development so that they will be able to be successful in their endeavors. We provide a supportive and positive work environment in order to attract and retain teachers and support staff of the highest caliber. We encourage the use of a variety of teaching styles, modern technology, and experiences outside of the classroom to meet the needs of students with different learning styles from different backgrounds. We seek to provide an enriching, professional, and comfortable environment for our students to learn and socialize in, and we offer a plethora of student services to assist our students with any issues they may encounter, whether those concerns are of an academic or personal nature. PROGRAMMATIC GOALS Upon completion of the Amlotus Intensive ESL (IEP) and TESOL(Teaching English to Speakers of Other languages) program, students will: IEP • Demonstrate ability to comprehend, interpret, and apply spoken and written forms of communication. • Demonstrate awareness of both explicit and implied meaning in extended spoken discourse. • Read with comprehension of both content and organization. • Recognize and utilize key vocabulary appropriate to a variety of situations. • Apply a range of language structures to interact effectively. TESOL • Demonstrate an understanding of how language is learned. • Identify current language teaching methodologies and their advantages and disadvantages, and effectively put practices into use. • Evaluate, select and use appropriate materials for language skill being taught. • Create a comfortable learning environment. LICENSING & ACCREDITING AGENCIES 1. Licensing Amlotus is licensed as a private career school governed by Bureau of Proprietary School Supervision (BPSS), New York State Education Department. 2. Accreditation The Amlotus Intensive ESL Program is accredited by the Commission on English Language Program Accreditation for the period April 2016 through April 2020 and agrees to uphold the CEA Standards for English Language Programs and Institutions. CEA is recognized by the U.S. Secretary of Education as a national accrediting agency for English language programs and institutions in the U.S. For further information about this accreditation, please contact CEA, 801 North Fairfax Street Suite 402A, Alexandria, VA 22314, (703) 665-3400. www.cea-accredit.org. 3. Certificate of Eligibility Amlotus, LLC, is authorized under Federal Law to accept nonimmigrant alien students. SCHOOL CALENDAR Amlotus programs run all year and utilize the academic calendar of quarters. Each session lasts 12 weeks. All new courses are open dependent on student enrollment. Minimum 5 students in enrollment are required to open a new course. ADMISSION INFORMATION Enrollment Dates New Students New students are allowed to register for the course by the end of week 2 after the course starts. After that, the registration for the course will be closed. In the event of the late enrollment, the student enrolled with missing hours must make up the missing hours by week 4. Continuing Students After meeting the graduation requirements for the course, the continuing students must register for the next course by week 2 of the scheduled course. Students who fail to register for the next scheduled course within the time frame described herein will be terminated by school within 30 days after the course starts. Admission Requirements FOR ALL STUDENTS To be enrolled in Amlotus Programs, Applicants must meet all of the following criteria: IEP (Intensive ESL Program) 1. AGE – Applicant must be at least 17 years of age by the time he/she starts school. 2. HSD or GED/TASC– A copy of High School Diploma or Transcripts that indicate a high school graduation date; College Diploma or Transcripts; OR GED (General Education Diploma) /TASC (If the document is not in English, it must be translated into English.) * Students who enroll for one-on-one English tutoring are not required to have a HSD or GED/TASC. TESOL (Teaching English to Speakers of Other Languages) 1. AGE – Applicant must be at least 17 years of age by the time he/she starts school. 2. DIPLOMA/TRANSCRIPT–Associate or higher Degree; OR college transcript that indicates the applicant is/was enrolled in 2nd year of college or higher. (If the document is not in English, it must be translated into English.) AMLOTUS LLC Page | 7

3. ENGLISH PROFICIENCY- This program requires English proficiency. For non-English speaker, the applicant must meet one of the following admission requirements; a. completion of our academic ESL (level 6) OR b. submission of one of the official scores-TOEFL iBT: 70, IELTS: 6.0, or TOEIC:635 Admission Requirements for International Students who need a student visa (F1) International students who need a student visa (F1) must submit all of the additional following documents, along with meeting the criteria for all students above 1. Affidavit of Support Form completed and signed OR Notarized Letters from parents and/or sponsor(s) from each financial provider which stats that they will be the financial providers for the student’s studies at Amlotus, LLC. 2. Original Bank Certified letter(s) or Statement showing enough funds (*) to cover the student’s financial needs for his/her duration of study at Amlotus including tuition, books and supplies, and living expenses. It must be valid for at least 3 months from the date of registration. (* Estimate of expenses for students enrolled in Amlotus Intensive ESL Programs for an academic year is $15,000.00 US dollars. If you have a dependent(s), the amount of $7,500.00 US dollars for a spouse, and (or) the amount of $5,000.00 US dollars for each child must be added respectively.) 3. Valid Passport – It must be valid for at least 6 months from the date of expiration. 4. International Student Application for SEVIS I-20 must be completed with applicant’s signature. 5. US $50.00 payment for the application fee. (Application fee is non-refundable.) Admission Requirements for transfer F1 students Transfer F1 International students must submit all of the additional following documents, along with meeting the criteria for all students above 1. Valid Passport – It must be valid for at least 6 months from the date of expiration. 2. F1 Visa and Form I-94 3. Signed Transfer Form 4. I-20 Form from a current school 5. Affidavit of Support Form completed and signed OR Notarized Letters from parents and/or sponsor(s) from each financial provider which stats that they will be the financial providers for the student’s studies at Amlotus, LLC. 6. Bank Certified letter(s) or Statement showing enough funds (*) to cover the student’s financial needs for his/her duration of study at Amlotus including tuition, books and supplies, and living expenses. 7. International Student Application for SEVIS I-20 must be completed with applicant’s signature. 8. US $50.00 payment for the application fee. (Application fee is non-refundable.) Credit for Previous Education or Training Because Amlotus is not a college, it neither grants, nor accepts student credits for previous education or training. Students with Disabilities Amlotus is a barrier free environment. Amlotus LLC will provide reasonable accommodations consistent with all applicable law to prospective students with a physical or mental disability and to students with needs related to their religious observance or practices. What constitutes a reasonable accommodation depends on the circumstances and thus will be addressed by the administration of Amlotus LLC on a case-by-case basis. Students with Special Needs AMLOTUS does not discriminate against individuals on the basis of physical or learning abilities and is committed to providing reasonable accommodations to qualified individuals with a disability, unless providing such accommodations would result in an undue burden or fundamentally alter the nature of the program in which the student enrolled. Individuals who are seeking Admission to AMLOTUS must provide documentation of their disability and meet with the Administrative Director prior to enrollment. The Administrative Director will determine whether or not the program of choice is appropriate, given the disability, and whether or not appropriate accommodations can be made. The decision of the Administrative Director is final. College Credit-Disclaimer Statement Licensed private career schools offer curricula measured in clock hours, not credit hours. Certificates of completion, i.e., school diplomas, are issued to students who meet clock hour requirements. The granting of any college credit to students who participated in and/or completed a program at a licensed private career school is solely at the discretion of the institution of higher education that the student may opt to subsequently attend. 9. ACADEMIC POLICIES AND PROCEDURES Attendance Requirements All students must maintain an attendance rate of at least 85% of classroom instructional hours for each course in order to graduate from the course. Any absences including tardiness up to 15 % of instructional hours do not need to be made up, but any absences from 16 % and below of instructional hours must be made up to meet satisfactory academic progress. Absences and Make-ups Any absences the student wishes to have excused must have written documentation as to the reason for the absence. - An excused absence means that the student has brought in and submitted to an administrator the appropriate documentation, and the excused absence is approved by a member of the Administrative Team. If an absence is excused, the student will be allowed to make up any missed work for full credit. It will be the responsibility of the student to make arrangements with the instructor to make up any work missed due to an excused absence. The student will have five (5) class days from the date of return to make up any work for full credit. If the student wishes to take more than five (5) days, he/she must make arrangements with the instructor. - An unexcused absence means that the student was absent and brought no documentation to the school as to why he/she could not attend classes. In the case of an unexcused absence, a student may make up work; however, the instructor reserves the right as to the issuance of full or partial credit. An instructor may elect to allow the student to turn in work for full credit, or the instructor may deduct partial credit for the lateness. - Excused or unexcused absences are not allowable absences; therefore, it is the responsibility of the student to make up the missed course work to reach attendance requirements for graduation. - Positive attendance and regular participation in classes are essential for academic progress. If a student who misses 15% of any course hours will receive an attendance warning letter and be asked to meet with the administration team to discuss how to improve his/her attendance. If a student’s attendance rate falls below 85% after the warning letter, the student will be placed on academic probation. The school will dismiss any student who at the end of such academic probation fails to raise his or her cumulative average to 1.5 and who fails to maintain attendance for at least 85 percent of the instructional hours offered during the probationary period. A termination letter will be sent to the student via an email or by mail and placed in the student’s file. AMLOTUS LLC Page | 10

Tardiness 1. If a student arrives in a classroom after more than 15 minutes after the class starts, he/she will be marked as “Late” (LT). If a student is continually tardy, the teacher will speak with the student. Excessive tardiness will not be accepted. Any student who misses more than 25% of a class lecture will automatically be marked absent for the rest of the class. While the student is free to stay in the class, he/she will not be marked present. 2. For attendance purposes, three tardies will constitute one absence. 3. Non-Attendance and lateness will not affect a student’s financial obligations to AMLOTUS LLC. 4. If a student is persistently tardy every week, he/she will be referred by the instructor to a member of the Administrative Team. If a student who misses 15% of any course hours will receive an attendance warning letter and be asked to meet with the administration team to discuss how to improve his/her attendance. If a student’s attendance rate falls below 85% after the warning letter, the student will be placed on academic probation. The school will dismiss any student who at the end of such academic probation fails to raise his or her cumulative average to 1.5 and who fails to maintain attendance for at least 85 percent of the instructional hours offered during the probationary period. A termination letter will be sent to the student via an email or by mail and placed in the student’s file. Leave of Absence 1. A request for an approved Leave of Absence must be submitted in writing, along with the reason for the leave of absence, and a reasonable expected timeframe of returning to school. The request must be received by the designated official of AMLOTUS in advance of the beginning date of the leave of absence (10 days prior to the leave of absence), unless unforeseen circumstances prevent the student from doing so. If a student does not request a leave of absence within a timeframe consistent with the ten (10) day consecutive absence policy, the leave may not be granted. 2. A student is entitled to Leave of Absence, not to exceed 5 months. In the case of a prolonged illness or accident, death in the family, or other special circumstances that make attendance impossible or impractical, a leave of absence may be granted to the student if requested in writing by the student or his designee. The approval such leaves of absence shall be in writing by the school director. No monetary charges or accumulated absences may be assessed to the student during a leave of absence. 3. The student must attest to understanding of the procedures and implications for returning or failing to return to school from a Leave of Absence. 4. When a student returns from a Leave of Absence, the student shall be placed in the instructional program at the point commensurate with the skill level retained by the AMLOTUS LLC Page | 11

student at the timing of his or her return. All students returning from a Leave of Absence will have their retention level assessed either through an oral or written evaluation instrument designed to measure the student’s level of retention of material covered in the courses they completed thus far. Documentation that such an evaluation occurred, along with the results, will be placed in the students’ permanent file. 5. Any student who does not return from an LOA for ten (10) calendar days after the return date and fails to notify the school will be terminated. Attendance Tracking Attendance is tracked in every class period by the instructor. There are several ways that the school checks, verifies and keeps attendance. 1. Student sign-in sheets – Students must sign their name in attestation on a student sign- in sheet to verify that they were in class. 2. The instructor then records the attendance (1) ten minutes after class begins on the attendance register form, and (2) ten minutes before class ends. 3. The instructor must initial the attendance register at the end of each class session. 4. In addition, when the course is finished, the licensed instructor must sign and date the attendance register in attestation on the last day of training for that month. 5. After each class, the instructor returns the attendance register to School Director. 6. If absent a first, second, and third time, that student will be contacted by phone and the results will be recorded on the Absence/Drop Record Form. 7. If absent a fourth and fifth time, the student will be contacted by telephone and in writing and will be advised of possible consequences. A copy of the same letter will be given to the student the next time the student comes to class, at which time the student must sign the Absence/Drop record form acknowledging receipt of the letter. Academic Probation will be implemented if the student is found in violation of the Standard Academic Progress (85% attendance) 8. If absent a sixth time, the student will be informed in writing that the course must be repeated and the student will be subject to paying additional tuition for the repeat. The Student will be formally withdrawn, and a refund calculation made based upon tuition liability and the last day of physical attendance. The student will not be able to repeat more than one course per program due to absences. Measures Taken When a Student Drops 9. If absent three times consecutively without notifying the school, the student will be considered dropped. The school will contact the student by telephone and in writing and formally advise the student of this status. Results will be recorded on the Absent/Drop Form. If appropriate, the student will be informed that a leave of absence status is available. The student will also be informed that the tuition paid will remain valid, unless there has been a significant financial or educational change to the program, in which case, extra tuition will be charged. The school will then request that the student respond in writing and provide reasons for withdrawal. If a withdrawal is requested, a refund will be calculated based upon the student’s tuition liability in AMLOTUS LLC Page | 12

accordance with his/her last day of physical attendance. The results will be recorded in the Absent/Drop Record Form 10. ACADEMIC PROGRESS Graduation Requirement In order to fulfill the requirements for graduation, a student must; IEP 1. Have attended at least 85% of class hours for each course he/she enrolled in; 2. Earn a minimum grade of C- (70%) in each course 3. Have met all financial obligations to the school, which includes full payment of tuition and any other fees in the enrollment agreements. * A certificate of completion will be awarded to all graduates. TESOL 1. Have attended at least 85% of class hours for each course he/she enrolled in; 2. Earn a minimum grade of B- (80%) in each course 3. Have met all financial obligations to the school, which includes full payment of tuition and any other fees in the enrollment agreements. * A certificate of completion will be awarded to all graduates. Grading System The grading system below is applied to all courses at Amlotus, LLC. Academic Probation Academic probation is an indicator that the student is not fulfilling the requirements of the curriculum. 1. An action for academic probation will take place if: a. IEP- The student’s final GPA for the course is below 1.7 (70%) – a letter grade of “C-.” b. TESOL - The student’s final GPA for the course is below 2.7 (80%) – a letter grade of “B-.” c. The student is absent more than 15 percent of the total number of instructional hours offered during each marking period of the student's program, excluding approved leaves of absence pursuant to page 11 of this catalog. d. The student fails to meet satisfactory academic progress at the end of any marking period, regardless of attendance. 2. The determination of probation is made by the Instructor and Academic Coordinator/Director of the school to place a student on academic probation for one marking period if the student would otherwise be dismissed pursuant to the requirements set forth in paragraph (1). 3. The school shall dismiss any student who at the end of such academic probation fails to raise his or her cumulative average to 1.5 and who fails to maintain attendance for at least 85 percent of the instructional hours offered during the probationary period. 4. Students are expected to attend all classes. Anything less than 85% of attendance and unsatisfactory academic progress- below cumulative GPA of 1.7 for ESL and 2.7 for TESOL would result in probation and register in a Progress Record/Report. Actions taken upon academic probation: a. The instructor and the Academic Coordinator/Director will have a meeting with the student, informing the student in writing that he/she will be on academic probation and completing a student counseling form that includes possible solutions, make-up projects, and tutoring schedules. The director and the student will both sign the student counseling form. The academic probation letter will be placed in a student’s file. b. The probation will last one quarter. During that time, the student must check in with the academic coordinator /director at least once per week and the academic coordinator/director will inquire about the academic progress from both the instructor and the student. c. The student and administrator will have a formal meeting at the end of the marking period. If the student’s GPA and attendance meet the requirement for the satisfactory progress, he/she will be removed from Academic Probation and stated on the following Progress Report/Record. If not, the probation period will be extended or the student will be terminated. The counseling form completed will be placed in the student’s file. 11. SCHOOL POLICIES Student Conduct & Suspension or Dismissal It is a key to maintain an effective, safe learning environment for academic success. All of Amlotus students are required to abide by Amlotus policies and instructions from school staff and management. 1. Act politely and professionally at all times. 2. Smoking is strictly prohibited on school premises. 3. Turn off cellular phone when class starts. 4. Keep school neat and clean – no spitting or littering on school premises 5. Attend every class on time and complete all assigned course work, course activities, and required quizzes and exams. 6. Bring course textbooks to class at all times. 7. Submit your own work. It is not accepted to plagiarize or copy another student’s work. 8. Only use the existing software installed in AMLOTUS LLC computers. Students are not allowed to install personal software on any computers or make any changes to computer systems. 9. Be responsible for any damages to computer equipment, furniture, or any other possessions of AMLOTUS LLC. If any damage occurs, the AMLOTUS LLC management will charge the responsible student and seek reimbursement for any damages. Immediate Expulsion 10. Student may be expelled immediately for the following incidents: stealing; seriously interfering with normal school operations; abusive behavior to school staff or other students; possessing a weapon; threatening the school staff, coming to school under the influence of alcohol, threatening or harming other students, or any similar behavior. 11. It is prohibited for any student to possess, use, or distribute illegal drugs or alcohol on school premises. Such behavior may be cause for immediate suspension or termination by a member of the Administrative Team AMLOTUS management reserves the right to suspend or dismiss a student for any conduct which seriously violates the Standards of Student Conduct. Prior to suspension or dismissal except the code for immediate expulsion, the AMLOTUS Administration shall AMLOTUS LLC Page | 15

give the student oral or written warning. If the student has no intention of correcting his/her behavior, a formal suspension or dismissal might be issued. Sexual and Other Harassment Harassment may be, but is not limited to: words, signs, jokes, pranks, intimidation, physical contact, violence or threat of violence, and racial, gender or ethnic slurs. Harassment is not always sexual in nature. Sexual harassment may include unwelcome sexual advances, requests for sexual favors or other verbal or physical contact of a sexual nature when such conduct creates an intimidating environment, prevents an individual from effectively performing his/her duties. Faculty, staff, and students may not engage in any form of verbal, physical, or emotional abuse, harassment, intimidation, violence, or threat of violence toward any student, visitor, client, instructor or staff member. Individuals wishing to report a complaint of this nature should request a copy of the Student Complaint Form. Submit the written complaint directly to the president /director. 12.TUITION AND FEES 1. Tuition and fees are due on or before the student’s first day of class. However, AMLOTUS may agree to accept signed agreements of installment payment as follows: [12 week course] • Initial payment: one third of the total tuition due before or on the first day of the class • Second payment: the second one third of the total tuition due by the end of the fourth week of the class • Third payment: the remaining balance by the eight week of the class. [10 week course] • Initial payment: half of the total tuition due before or on the first day of the class • Second payment: the remaining balance of the total tuition due by the end of the fifth week of the class In the event the installment payment plan occurs, $100 installment processing fee will be charged per course tuition. Students can pay off the remaining balance any time before the due date. 2. The course registration fee (*below) will be applied one time and is non-refundable and non-transferable. Such date shall be the earliest of (i) the date on which the student gives written notice to the school or (ii) last date of physical Attendance. If a student has failed to attend classes for a period of fourteen (14) calendar days, the school shall send by regular mail a notice to the student that the student shall be deemed to have withdrawn from the program if the student does not notify the school to the contrary within twelve days from the date on which the letter is sent. If the student fails to respond within such twelve-day period, the student shall be deemed to have withdrawn pursuant to Section 5002 of the Education Law. 15.SPACE, EQUIPMENT, & FACILITIES AMLOTUS, located at 500 8th Ave. Suite 909, New York, NY 10018, is placed in the center of Midtown Manhattan. Amlotus Campus is easily reachable by New York City subway and rail lines and buses from outside areas. All of the stations- A, E, F, N, Q, R, 4, 5, and 6 subway lines and LIRR, New Jersey Transit, and Path – are positioned within walking distance of the School. AMLOTUS occupies a 5-room suite with approximately 2,500 square feet of floor space, which is divided into 3 classrooms, 1 administrative office, an open reception area, and multiple school agent booths. Also, computer lab is available on the school premises. The school offers contemporary equipment as in essential part of each course. The equipment used for learning consists of electric white boards, computers, relative software, scanners, and printers. TESOL (Teaching English to Speakers of Other Languages) (180 hours) Course For non-English speaker, the applicant must meet one of the following Requirement: admission requirements; English a. completion of our academic ESL (level 6) OR Proficiency b. submission of one of the official scores Required Course - TOEFL iBT: 70, IELTS: 6.0, or TOEIC:635 Description TESOL program is to provide individuals who are interested in teaching ESL or EFL foundational skills and strategies to be successful in the classroom. Course Goal Course participants are required to have a high level of fluency, both orally and written, in English. The program equips them with a foundation in language acquisition, ESL methodology, and prepares them for further studies or professional development. Demonstrate an understanding of how language is learned. Identify current language teaching methodologies and their advantages and disadvantages, and effectively put practices into use. Evaluate, select and use appropriate materials for language skill being taught. Create a comfortable learning environment. Evaluate own teaching effectiveness. 17.FINACIAL ASSISTANCE Financial Aid is currently not available at Amlotus, except a limited number of Amlotus issued scholarships. For more information regarding Amlotus scholarships or financial aid, please contact the School at 212-912-0100. 18.STUDENT SERVICES Amlotus offers a variety of student services to its students. Orientation Once students have been admitted, the prospective international students are given a Pre- arrival packet that includes information about academics, lawful immigration procedures and statues, and life in New York through USPS mail or email. On-site Student Orientation for all new students is conducted approximately one week prior to the students’ first day of class. Based on the individual’s academic objectives, the student is offered advice on curriculum and course selection by the Office Manager. The purpose of orientation is to better familiarize students with the school environment and to provide important information necessary for successful course completion. This is also an opportunity for students to ask questions about the school, curriculum, and hand in any final registration documents. Family members are welcome to attend. During the orientation, a copy of School Catalog will be distributed to all prospective students. In addition, upon request prospective students will receive a digital copy of School Catalog via en email. The Office Manager will review the School Catalog with the student and AMLOTUS LLC Page | 27

answer any questions the student may have. The student must sign a School Catalog Acknowledgement Form acknowledging receipt and comprehension of the information contained in the School Catalog. Consultative Services Amlotus has office managers/directors available to answer policy and regulation questions. The managers are available from 9:30 am to 6:00 pm on Monday through Friday, and can be reached by telephone at (212) 912-0100, or via email at [email protected]. Emails are checked on a daily basis. Students will also be provided academic advising/testing, and advising on college admissions upon request and given a list of instructors’ names, email addresses and courses taught. . Student IDs AMLOTUS students will be issued a photo ID upon registration. To request an ID, students should contact the Office Manager as soon as they are registered for classes. Students should carry their IDs with them at all times while in attendance at the school. AMLOTUS student IDs entitle students to any applicable benefits which students receive. Applying for the first student ID card will be at NO cost. If the ID card is lost, each additional ID will cost $10.00. The student ID entitles the student to discounts with any company that offers discounts to students currently enrolled in school. Lab Hours The computer lab (Classroom III) is available for student use anytime there are no classes being held. Students may not download any software onto these computers, but are free to use these computers for independent and classroom practice. Computer & Tutorial Lab (at No Cost): Normally, tutoring is available for 4 hours per week in the Computer Lab with an experienced Tutor to assist students with lessons and studies. Policy for Granting Credit for Previous Education and Training Because Amlotus does not give credits for coursework performed, it does not grant any credit for previous training and education. ESL students may skip levels and be started in a higher level of ESL if they demonstrate via an entrance test that their English level is advanced. Student Records Keeping Period Student Enrollment Agreements and payment records shall be kept for a maximum of 7 years. Student final grades shall be kept for a maximum of 20 years. Transcripts for Students Within 20 years of the date of his/her graduation, a student may request that AMLOTUS supply an official (sealed) or non-official transcript. A Transcript Request Form must be completed by the student. There is a $10 charge for each transcript. Lost and Found All lost and found items are to be reported to AMLOTUS Agents, and these items will be kept in the “Lost & Found” box. A student should immediately contact an Agent for information if he/she realizes any item has been lost at the school. After 30 calendar days, the school will not be responsible for keeping these items. Housing At the time of this publication, AMLOTUS does not supply student housing. However, students may contact an AMLOTUS Agent to find out about NYC housing information, such as related websites, etc. Health Insurance AMLOTUS does not provide students with any type of health insurance. We strongly recommend that each student obtain an individual insurance policy in case of illness or emergency. There are serious consequences that can be associated with lack of health insurance. During Orientation, each student will be given a referral list which will contain the names and addresses of places where he/she might be able to obtain health insurance. If a student needs additional referrals, the Office Manager will be glad to assist the student with that. AMLOTUS LLC Page | 29

19.STUDENT COMPLAINT PROCEDURES AMLOTUS encourages all students to openly discuss any questions or complaints they may have related to school’s academic affairs and management with the staff of AMLOTUS. 1. If a student has a complaint, he/she should first inform the office manager or an instructor. The office manager or the instructor shall discuss the problem with the student and try to find an appropriate way to resolve it. If the problem is not resolved, the agent (or student) can report the problem to the Academic Coordinator. 2. The Academic Coordinator will arrange a meeting with the student and any related AMLOTUS staff and try to resolve the problem. In case the student is still not satisfied with the solution, he/she should contact the Administrative Director/President. 3. The Administrative Director will hold a meeting with all parties involved in an attempt to solve the student’s problem. 4. If the student still is not satisfied, he/she right to send the complaint in writing to New York State Education Department: Bureau of Proprietary School Supervision New York State Education Department 116 West 32nd Street, 5th Floor New York, NY 10001 20.DISCLOSURE STATEMENT The student should be aware that some information in the catalog may change. It is recommended that students considering enrollment check with the school director to determine if there is any change from the information provided in the catalog. In addition, a catalog will contain information on the school’s teaching personnel and courses/curricula offered. Please be advised that the State Education Department separately licenses all teaching personnel and independently approves all courses and curricula offered. Therefore, it is possible that courses/curricula listed in the school’s catalog may not be approved at the time and courses/curricula that a student enrolls in the school or the teaching personnel listed in the catalog may have changed. It is again recommended that the student check with the school director to determine if there are any changes in the courses/curricula offered or the teaching personnel listed in the catalog. 21. STATEMENT The failure of a student to immediately notify the school director in writing of the student’s intent to withdraw may delay a refund of tuition to the student pursuant to Section 5002(3) of the Education Law.
