-ESRD initiating HD access (AVF, AVG, HC) between January 1, 2006, and December 31, 2014 -870,571 patients -29.8% (n = 259,686) developed sepsis. -HC (31.2%) and AVG (30.6%) were associated with a higher number of septic cases compared with AVF (22.9%; P < .001). -The incident rate of sepsis was 12.66 episodes per 100 person-years (Locham et al., 2021)
Patients with sepsis -Older -Higher number of comorbidities including ❖ Obesity ❖ Congestive heart failure ❖ Peripheral arterial disease ❖ Diabetes (Locham et al., 2021)
-Retrospective study conducted from August 2012 to July 2019 in hemodialysis patients with BSI at Kawashima Hospital. - 174 patients 55 (30.9%) had the infection from unknown origin. - The most frequent bacterium was “Staphylococcus aureus”. - Low serum albumin level of 2.5 g/dL is a cut off value (Shima et al., 2021)

Viral hepatitis in patients on hemodialysis -HBV can remain infectious on environmental surfaces for at least 7 days. -HCV can live in dried blood spots to be infectious from days to weeks

การป้ องกนั การคดั กรองอยา่ งสม่าํ เสมอ -ผปู้ ่ วย anti-HCV เป็นลบ และ ALT (alanine aminotransferase) ปกติ ตรวจทุก 6 เดือน -ผปู้ ่ วย anti-HCV เป็นลบ และ ALT สูงผดิ ปกติ ไม่ทราบสาเหตุ ใหต้ รวจ anti-HCV ซาํ ถา้ เป็นลบใหต้ รวจ HCV-RNA - ผปู้ ่ วย anti-HCV เป็นบวก แนะนาํ การป้ องกนั การติดเชือตาม มาตรฐาน ไม่ตอ้ งแยกผปู้ ่ วย และตวั กรองนาํ กลบั มาใชไ้ ด้ - ผปู้ ่ วย anti-HCV เป็นบวก และ ตรวจพบ HCV-RNA ใหต้ รวจ anti-HCV และ ALT ในผปู้ ่ วยที่มีผลเป็นลบ ทุกราย ทุก 1-3 เดือน จนไม่ พบผปู้ ่ วยรายใหม่ จึงกลบั ไปคดั กรองทุก 6 เดือนตามปกติ

WHEN YOU SHOULD PERFORM HAND HYGIENE -Before & After you touch a patient -Before you inject or infuse a medication -Before you cannulate a fistula/graft or access a catheter -After you touch blood, body fluids, mucous membranes, wound dressings, or dialysis fluids -After you touch medical equipment or other items at the dialysis station -After you remove gloves Remember: perform hand hygiene between each patient or station
USE PERSONAL PROTECTIVE EQUIPMENT (PPE) CORRECTLY -Wear gloves, a gown, and/or face protection when you think you may come into contact with blood or other potentially infectious materials -Change gloves during patient care if the hands will move from a contaminated body-site to a clean body-site -Remove gloves after contact with a patient and/or the surrounding environment (including medical equipment) Do not wear the same pair of gloves for the care of more than one patient
