WELCOME MESSAGE FROM OUR CHAIRMAN & PRESIDENTWelcome to Friends of Golf’s 37th Annual Charity Golf Tournament. This year’s honoreesprovides us with a platform to honor the amateur golfer. John Cook, Jerry Pate and BrysonDeChambeau have all been United States Amateur Champions. This championship haslong been recognized as the toughest championship to win because of the intense qualifyingroutine followed by winning six consecutive matches. In addition to many other wins by all of ourhonorees, Cook won the NCAA Team Championship at Ohio State in 1979; Pate won theU.S. Open in 1976 and DeChambeau won the NCAA Individual Championship last year.Amateur golf is what FOG is all about. Since our founding in 1979, we have had singular focus: Sal Cimboloto support amateur golfers to play the great game of golf. Our support goes to many collegeand high school programs across the country. We are also providing significant support to Chairman & CEOunderserved juniors in southern California and beyond through our partnerships with SCGAJunior, The First Tee and the PGA of America.We also honor Rick Taylor in 2016. As President & CEO of Konica Minolta, Taylor has been aTitle Sponsor of the FOG tournament for almost 10 years. He is certainly deserving to receivethe Ken Venturi “Spirit of Giving” Award.With you, and leaders like Rick Taylor, we will continue to increase our support of amateur golf aswe approach $8 million raised to date. There is plenty to do to give youngsters the chance to playthis great game of golf and learn all of the valuable life lessons that it has to teach.To all participants, volunteers and sponsors, we wish you a great day on February 22nd. President & Tournament ChairmanFOG LOSES KEY MEMBER OF ORGANIZATION’S SUCCESS Joyce Haack Throughout FOG’s 37 year history, many individuals have contributed to the success of the organization and its mission. However, there was only one individual referred to as “the heart and soul of FOG.”FOG Employee (37 Years) Joyce Haack was there from the beginning and for many years was FOG’s only employee. Being the only employee for an all-volunteer non-profit organization can be taxing, but when those volunteers are made up of billionaires, a Major League Baseball Commissioner, a Heisman Trophy winner, Captains of Industry and other high profile individuals, keeping them on task can be a monumental job. Joyce never minded being in the background while allowing others to bask in the limelight. She could always be counted on for two things; kindly making sure FOG’s board members followed through on their commitments and making sure they had the resources and support required to accomplish their duties. Joyce was everything to FOG. Many have wondered why Blue Jay, CA is listed as FOG’s address. The simple answer is because the small village in the San Bernardino Mountains was where Joyce’s home was located. She handled all of FOG’s correspondence and mailings, all aspects of tournament registration each year, and on Tournament Day, she was there to insure everything ran smoothly. Until her health began to fail, Joyce organized and attended all Board and Executive Board meetings, took detailed minutes and made sure everyone received a copy promptly. When an organization touts that over 90% of the funds raised go directly to beneficiaries, you know paid staff has its act together. Joyce embodied an era when an “executive secretary’s” efficiency, knowledge and adherence to protocol was critical to her boss’ success. FOG was extremely fortunate to have Joyce from the organization’s first day and to be its heartbeat for the next 37 years. Joyce passed away on January 2nd, 2016, after a 9 month battle with cancer. She will be missed by all. 3

In the event of any conflict between the terms and conditions setforth herein and those of the prize donors, the terms and conditions set forth by the prize donors shall take precedence. By participating in the silent and/or live auctions YOU AGREE TO ACCEPT, WITHOUT LIMITATION OR QUALIFI- CATION, ALL OF THE TERMS AND CONDITIONS OF THE AUCTIONS AS WELL AS THE TERMS AND CONDI- TIONS PROVIDED WITH THE DESCRIPTIONS OF THE ACTUAL AUCTION ITEMS displayed on the BidPal devices. Before actually bidding please carefully review the descriptions of any auction items you intend to bid on as some components of an offered item may have changed between the time the booklet describing the auction items was printed and the time of the auction. Certificates provided by airlines, hotels, land and cruise companies were donated to Friends of Golf for our auctions and are considered to be non-revenue certificates and, as a result, you are not permitted to go online to make your reservations for flights, rooms, cabins, tee times, etc. All auction participants who are successful bidders of items involving travel, hotels, resorts, etc. are strongly encouraged to utilize the services of Classic Vacations, a world-class travel company that specializes in arranging all of your trip planning/reservations in connection with your prize(s), from the time you leave home until you return. It provides this service at no charge. Please review all of the terms and conditions on your certificates relative to your prize before contacting Classic Vacations and, please understand that you must be flexible with your travel dates. All airlines, hotels and cruise lines will charge applicable taxes, fees, and Port Charges based on the city, state and/or country, all of which is the winning bidder’s responsibility. It is very important that you understand that lost or stolen certificates will not be replaced by the vendors or Friends of Golf. You must have in your possession the original certificate(s) when you check in to the airline, hotel, cruise, etc. Winning bidders in the live and silent auctions must pay for their item(s) by reporting to the auction check-out desk on-site immediately following the close of the silent auction. Acceptable methods of payment in U. S. currency include cash, check, or the purchase price may be charged to the bidder’s personal or business Visa, MasterCard, American Express or Discover credit card. The winning bidder becomes the owner of the item(s) at the close of the live and silent auctions when he or she pays for the item(s) and from then on bears the risk of damage to or loss of the item. Any dispute, controversy or claim arising out of or relating to the Friends of Golf Tournament auctions shall be settled by binding arbitration in accordance with the commercial arbitration rules of the American Arbitration Association. Any such controversy, dispute or claim shall be arbitrated on an individual basis, and shall not be consolidated in any arbitration with any claim, dispute, or controversy of any other party. The arbitration shall be conducted in Santa Monica, California, and judgment on the arbitration award may be entered into any court having jurisdiction thereof. Any award rendered in any such arbitration shall include an amount equal to the reasonable costs and expenses, including reasonable attorneys’ fees, incurred by the successful party. FRIENDS OF GOLF IS PROVIDING THE AUCTION ITEMS FROM DONOR/VENDORS ON AN “AS IS” BASIS, WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF TITLE OR MERCHANT ABILITY OR FITNESS FOR A PARTICULAR PURPOSE, OTHER THAN THOSE WARRANTIES WHICH ARE IMPOSED BY LAW AND INCAPABLE OF EXCLUSION, RESTRICTION OR MODIFICATION UNDER THE LAWS OF THE STATE OF CALIFORNIA APPLICABLE TO THESE TERMS AND CONDITIONS. U. S. citizens must have a passport valid for six (6) months from travel date. Please confirm the entry requirements of each destination either through Classic Vacations or on your own. Citizens of other countries need to inquire about visa requirements with the consulates of the corresponding destinations. It is solely the bidder’s responsibility to ensure that all documents needed are complete and current. International tickets cannot be issued until the airline receives a copy of the name page of the passenger’s passport, along with TSA required information. Privacy policy: Friends of Golf does not disclose any of the personal information provided to it by participants in its golf tournaments and/or auctions to anyone, for any purpose, with the exception of its employees who may only use such information to contact past participants regarding future tournaments, as well as sending its Semi-Annual FOG Horn Newsletter to the same. 17
