We work hard to maintain our qualifications including UKAS Certification for OHSAS 18001, ISO 14001 and ISO 9001 including NHSS16, Safecontractor, Constructionline, CHAS &Achilies. KG003 Rev 3/2019

1) Scope of Works The object of this Method Statement is to present how the asphalt will be laid at the above site 2) Personnel for this project Andy Holborn Mobile no – 07717 794658 T.B.C Mobile no – As Per Coversheet Operations Director T.B.C Mobile no – As Per Coversheet Contracts Manager T.B.C Mobile no – As Per Coversheet Site Supervisor Kevin Gregory Mobile no – 07701 320471 Site Foreman H&S Manager (off site) 3) Sequence of works (methodology) Machine Lay & Hand lay works- A supervisor will visit site 48 hours prior to commencement of work to complete a survey of agreed Area. He will liaise with the site engineer to agree on the measure of the area along with a comprehensive dip to ensure the correct formation of sub base is achieved. A copy of the dip sheet will be left with the site engineer. On arrival to site our supervisor, foreman and operatives, will then attend a site induction to make them fully aware of the site rules and regulations along with welfare facilities supplied by Barnwood Construction. The foreman and operatives will complete a pre check of all plant and equipment to ensure it is in safe working order prior to commencement of works. This is recorded on our Daily Plant Inspection forms (DPI01 form) and copies can be taken by if required. The supervisor and foreman will then brief his team on the details within this document and walk the site to discuss and agree on the best laying practices. The gang will consist of 7 surfacing operatives including a Foreman (These numbers could vary at short notice due to sickness / holidays). There will also be a Supervisor in attendance throughout the majority of the works but during times when the supervisor is not in attendance the Foreman will take full responsibility for the conduct and Safety of his team. Each operative will hold a CSCS card and either NPORS or CPCS Cards for the plant they operate. Segregation of works and TM will be the responsibility of the Client. The Supervisor / Foreman should liaise with the client to ensure good communication is maintained throughout the works. The sequencing of all work will be agreed with the client before the work begins. This Statement will NOT cover working around overhead cable or pylons. If either of these hazards are identified a Method statement for working near electricity should be issued by the Gibbs Surfacing H&S Manager (see section 4 Risk Assessment). The material will be delivered to site by 6/8 wheeled insulated Lorries. The agreed supplier will have been advised that all delivery drivers will be required to attend a delivery driver’s induction. A trained and competent banksman will direct the lorry to an agreed waiting area prior to laying the material. Segregation of works will be the responsibility of the Client. Where tack coat is required, we will supply a K140 bitumen emulsion that will be laid by hand held mechanical sprayer. KG003 Rev 3/2019

Machine Lay process - On commencement of machine lay work, a trained and competent banksman will direct the driver onto the front of the paving machine so that the materials can be loaded directly into the hopper. Regular visual checks of the material will be undertaken to ensure consistency and quality of material. Temperature of the material will also be monitored using a probe thermometer. Materials found to be below recommended minimum laying temperatures will be returned to quarry. Where applicable, subject to site conditions, the paving machine will push the lorry whilst still depositing the material into the hopper. If there are constraints on site then the lorry will lower its bed and pull off the front of the paving machine until all material is laid. All operatives must stand clear from the side of the paving machine when any moving parts are being lowered or raised. The above process will then repeated until all material from the lorry is deposited into the hopper. The material will then be compacted by a tandem roller and pedestrian hand roller where necessary. Once the unloading is completed, the driver will exit the site using the same entry route unless otherwise instructed. Hand Lay process - On commencement of hand lay work, the lorry will remain stationary and raise its insulated bed in preparation for use of the chutes. A particular care and attention need to be paid by the operatives to ensure the driver has secured the tailgate prior to using the chutes. The operative will then raise the chutes on the lorry and deposit material into the wheelbarrow. The operative will then deposit the material in the agreed area ready for hand laying. This process will be repeated until all material has been laid. The material will then be compacted by a pedestrian hand roller and whacker plate/hand tamper where necessary. Once the unloading is completed, the driver will exit the site using the same entry route unless otherwise instructed. Completion At the end of each working day, the foreman and operatives will leave all working areas clean and tidy and free of safety hazards. All plant and machinery will be left in a designated area where keys will be removed for security and safety purposes. 4) Breaking Ground If there is any requirement to break ground a permit should be issued by the main contractor and will include detailed information and STATS. A copy of this information will be given to the team involved with breaking ground in addition to this, when breaking ground at depths greater than 150mm in asphalt or concrete surfaces, a full survey with ground radar and or cat scan will be undertaken by the main contractor to identify the location and depth of underground services. The locations of underground services will be identified and the surface marked as appropriate. 5) Risk Assessment See attached risk assessment for each of the activities & processes identified in the above methodology. N.B. Risk Assessment and Method Statement are subject to change should the supervisors pre start visit highlight hazards not already considered in these documents. Any updated documents will be sent to the client upon completion. In the event of overhead cables there will need to be a separate Method statement issued and a rolling bed vehicle to be booked for delivery of materials. The booking of such vehicles requires notice and Gibbs Surfacing Ltd will not be held responsible for any delays caused by the Clients failure to notify us of electrical hazards / overhead cables / Pylons on site prior to our engagement. There are no aspects of our work that should require a separate permit to work i.e. confined space, hot works, ladders etc. KG003 Rev 3/2019

6) Access / Egress No access / egress problems noted at present. When site supervisor attends site prior to works any problems will be noted & reported. Method statement will be updated accordingly. Gibbs Surfacing site staff will work in line with Barnwood Construction Traffic Management rules. 7) Plant & Equipment Safety belts to be worn at all times where provided and hazard warning beacons to be operated at all times. Notice of conformity will be provided for the plant Machine Lay – 1 x Midi Paver 1 x 360 Excavator 1 x 6t Dumper 1 x BOMAG 161 / 120 / 138 Deadweight roller 1 x Pedestrian hand vibe roller 1 x CAT Skid steer 1 x Supervisors car derived van 2 x Crewcab vans – Operatives 1 x PMB 50 Tanker (If required)(see separate assessment “Spray Tanker” should this equipment be required) Hand Lay – 1 x pedestrian hand vibe roller Wacker plate Hand held tamper Wheelbarrows Squeegee boards Hand held rake Shovels 1 x Supervisors car derived van 2 x Crewcab vans – Operatives 8) Materials Gibbs Surfacing Ltd will ensure that all materials are stored in a safe location during working hours. Site security out of hours will remain the responsibility of the main contractor. All materials can be supporting with CE & DOP certification upon request of an O&M pack. O&M Packs are only available after completion of the work to allow for specific material certification. (requests to: [email protected] ). COSHH Data sheets should accompany this document. All operators are aware of their responsibilities towards COSHH regs 2015 and are trained to carry and use spill kits in each vehicle. There are no significant hazards attached to our substances. All substances are stored within our vehicles and/or an accompanying Tanker (PMB50 spray work). A breakdown of materials used for this contract are as follows: AC32 HDM Base 40/60 AC20 Dense Bin 40/60 AC10 Close Surf 40/60 H/S AC6 Dense Surf 40/60 L/S 9) Supervision 75% of our Supervisors currently hold a SMSTS qualification and the remainder have SSSTS. The supervisor for this project will be made aware that he needs to attend a separate induction. The supervisor or foreman will also make a daily inspection prior to work commencing. The Foreman of the gang also holds a SSSTS certificate. All site staff are supervised by the Supervisor or Foreman. Once our Supervisor has discussed all aspects of the job as detailed in (3) he will hand the job over to the Foreman. The Foreman remains with the gang all day and ensures that H&S and Quality are adhered to at all times. 10) Emergency arrangements As advised in site inductions. Verbal & telephone communication used as main form of communication First Aid – First aid and firefighting equipment are kept in the van at close range so it is easily accessible in case of emergency. Gibbs’ Site Supervisor/Foreman will inform the H&S Manager (Kevin Gregory) /senior member of Gibbs management, and Barnwood Construction Site Manager in the case of ANY accident, illness or emergency which may occur in the course of work. In the event if an accident/injury on site First Aid will be administered and the emergency services summoned where required. Any such occurrences are noted within our accident book and investigated. Near misses will be reported in the same way. 11) Housekeeping Workers on site are responsible for clearing the working area and ensuring it is left in a tidy, safe manner at the end of each working day. Any surplus material will be removed from site and disposed of in the correct manner and any plant will be left in a safe, secure place ready for use the next day or to be collected. 12) Quality Control Suppliers of materials are ISO9001 approved. Visual checks of materials when delivered are made along with temperature checks. Checks will be made for high levels of grit and coating of aggregate. 13) Environmental aspects No hazardous waste will be produced during this work sequence. Any surplus material will be returned to the quarry or disposed of in accordance with current legislation. Drip trays are used when filling up with fuel. In the event of any spill, spill kits are kept with each surfacing gang and will be used in accordance with their instructions. There are no significant hazards attached to our substances. All substances are stored within our vehicles or an accompanying Tanker (PMB50 work). Our processes will not create dust unless a disk cutter is required. In these circumstances we will use water suppression to limit the dust and a face mask will be worn by the operator. 14) Programme This work is scheduled to take place as follows: Visit 1. 14th April 2020 – 5 Days Machine lay base / binder course to spine road. The remainder of the phasing will be discussed at the pre-let meeting arranged for 16th March 2020 09:00hrs. The area of work is to be agreed with Barnwood Construction site supervisor during our Supervisors initial visit or upon arrival. The sequencing of work should be agreed with the Client at an early stage and any changes to the plan should be communicated to either our Supervisor or Forman as soon as possible. Supervisors and foremen are encouraged to communicate regularly with the client to support a smooth transition throughout the work. Gibbs Surfacing Ltd would expect the Client to sequence other operations around the vicinity in a way as to ensure that our gangs have a clear and safe work space at all times. KG003 Rev 3/2019

15) Training Gibbs Surfacing Ltd has provided training for the following disciplines. a) Safe refuelling of Machinery b) Safe use of equipment and machinery c) Safe Operation of Bitumen Boilers d) Portable Tools (Saw, Hammers, Drills etc). e) First Aid. f) Vibrating Tools g) Generators All staff on site have current CSCS card and operatives have CPCS cards for the plant they are using Site Supervisors have SITE MANAGERS SITE SAFETY CERTS (SMSTS) & Foremen have CONSTRUCTION SITE SUPERVISOR’S SITE SAFETY CERTIFICATES (SSSTS) 16) Personal Protective Equipment All operatives will be supplied with the following personal protective equipment. N.B. Flat soled boots must not be worn when driving plant due to the limited grip offered by the sole. a) Honeywell Metatarsal Safety boots (EN ISO 20345) b) JSP Skyworker Concept Safety Helmets (EN12492, EN50365) c) High visibility clothing (EN471) d) Gloves – Portwest A320 Dexti-Grip Glove (EN420, EN388) e) Safety Glasses (BS EN166.1. F) f) High Impact Safety Goggles (BS EN166.1.B) g) Ear defenders (EN352-3) h) Dust Masks - 3M AURA 9332 (EN 149:2001 FFP3) HAVs Trigger times are as follows: Breaker – 99 mins per 8 Hrs Vibe plate – 148 Mins per 8 Hrs Disc cutter – 69 Mins per 8 Hrs Hand Vibe – 99 mins per 8 Hrs Gibbs Surfacing Limited has given their staff permission to remove Safety Helmets/Hard Hats for short periods of time, so they can get relief from hot working conditions. This is for welfare of our site staff to ensure they do not overheat or suffer from heat exhaustion. They are also given permission to remove Safety Goggles / Safety eyewear when not working or during heavy rain to ensure they have clear, unobstructed vision when walking / working on site. These exceptions only apply if permission is granted by Barnwood Construction Site Manager. KG003 Rev 3/2019
