63Two weeks of camp: $475.00 Transportation: $40.00 (Includes $30 registration fee) Make checks/money orders payable to New Horizons Ministries, Inc. $30 registration fee is non-refundable and non-transferable. Payment by personal check WILL NOT be accepted after 7/2/16. $15.00 fee for checks returned by the bank for any reason. Requests for any financial assistance must be submitted by 7/2/16.Registration, Contract, Health and Release forms should be submitted by July 18, 2016 (seeinsert). Children will not be allowed to attend camp without these forms.TRANSPORTATION available ONLY if r equested. 4 5 MEDICAL AND OTHER MANDATORY FORMS WHAT TO PACK….. Current medical and immunization history is Checklist: required by law. Campers are not allowed on campgrounds without: Bible, Notebook and Pen  Flashlight (w/fresh batteries) Sleeping bag or sheets and pillow case  Completed Registration, Health (with Immunization rec- Towels and washcloths - at least 2 ords) and Consent Forms Knee Socks, Long Pants, Long Sleeved Shirts and a Hat All prescriptions and over-the-counter medications must be Sports/play clothes (no short shorts, see -through/halter tops or short shirts) given to the nurse on the first day of camp.  Sneakers  One piece swimsuits only  Insect repellent & Sunscreen  Toiletries (soap, toothbrush, toothpaste, deodorant) + bag to contain themThe camp program is filled with exciting activities, many of which will take the Knee Socks, Long Pants, Long Sleeved Shirts and a Hatcampers away from the main building or away from the campsite. For this reason Dress Attire for closing ceremonieswe ask parents to adhere to the following: Please label everything with your child’s name (use permanent marker) . We will notARRIVAL AND DEPARTURE be held responsible for any items lost or missing if not labeled.ARRIVAL: Parents bringing children to camp on the first day, arrival time is 4:00 P.M.; DO NOT BRING TO CAMP….NOT BEFORE.DEPARTURE: Camp is officially closed at 1:00p.m., on August 5th, 2016, if picking up  No CELL PHONES or other electr onic gadgets, e.g. TV, r adio, CD player s, etc.children from the camp site all children must be picked up no later than 2:00p.m.  No Skateboards, playing cards, fireworks, pets, food or snacks, *alcoholic beverages,As noted on the registration form, your child will be released only to the person *tobacco, *drugs, *weapons. (*Possession of any of these items will result in immediatelisted. Should this information change please contact New Horizons Ministries, Inc. removal from camp.)with the updated information before the last day of camp.  No see-through or net shirts, outfits with bare midriff, short shorts, or mini skirts.  Money is not needed at camp and not allowed in the dorms. If found it will be held by the camper’s counselor or registrarVISITS AND PHONE CALLS ALL CAMPERS ARE REQUESTED TO .....Visits and phone calls are not encouraged except on Family Day, Saturday, July 30 (see  Dress modestly at all times in comfortable and casual clothes.
