VISA REQUIREMENTS JAPAN For Tourism 1. Fill out visa application form - original sign 2. two (2) photos (4.5 cm x 4.5 cm)–photos must be pasted onto application form-original 3. Phil. Passport - original 4. Certified true copy of birth certificate issued by the Philippines Statistic Authority (PSA). In case of no record of birth at PSA, birth certificate by city hall and authenticated by PSA as well as certificate of non-record of birth by PSA. The applicant, who can submit the passport (including old passport) showing the fact that he/she obtained Japanese visa and visited Japan in the past, will exempted to submit birth certificate.
