Credential ManagementHealthcare staffing is a dynamic environment in which required credentials and documentation expire everyday. Consequently, employee records are reviewed on a regular basis to ensure their accuracy and integrity.This practice facilitates the timely renewal of required documentation. • Health Carousel is proactive in monitoring credential expirations. Credentials are monitored utilizing Microsoft Dynamics CRM that tracks expiration dates of documents and credentials with Contact Compliance Requirements (CCR). These CCRs are monitored on a weekly basis to assure all credentials are current. At least 45 days prior to expiration date, a Credentialing Specialist contacts the HCP via phone and email to inform them of expiring documents. The credentialing specialist or assigned Health Carousel representative will follow up every 7 days until the updated credential is submitted. • The records of all employees are reviewed by Health Carousel’s Credentialing Department to identify expired documents, before they are extended on his/her current assignment or before placement on a new assignment.Privacy, Confidentiality and Security of InformationHealth Carousel is committed to protecting the privacy, confidentiality and security of personal information ofits employees. This policy is designed to assure compliance with applicable state and federal laws andregulations.HIPPA Compliance: Agency shall at all times safeguard the integrity, security, and confidentiality ofindividually identifiable health information, as defined in 42 U.S.C. Section 1320d(6) (“Health Information”), towhich it has access by virtue of client Agreement/s. To accomplish this requirement, Agency shall maintainreasonable and appropriate administrative, technical, and physical safeguards as specified in 42 U.S.C Section1320d-2(d)(2).Health Carousel shall familiarize its employees with the privacy and security policies of client facilitiesapplicable to Facility’s Health Information. Health Carousel and client facility shall promptly to one another anyunexpected incidents regarding any breach of integrity, security or confidentiality of Hospital’s HealthInformation.OwnershipHealth Carousel shall maintain ownership of employee records. These records may be removed from HealthCarousel’s jurisdiction only by: 1. Subpoena 2. Court Order 3. Applicable statuteGuidelines for Maintaining Privacy and Confidentiality of InformationAlthough it is the desire of Health Carousel to provide authorized third parties with information wheneverrequested, it is our responsibility to control the release of information to protect the privacy and confidentiality 50 OPERATIONS MANUAL | v13018

of employee and/or corporate information. Health Carousel has therefore developed the following guidelines incompliance with the applicable laws and statutes for maintaining the privacy and confidentiality of records: • All applicants are required to sign the Background Authorization in order for their application to be processed. This enables Health Carousel to obtain necessary data from third parties to verify background including education verification if applicable information provided by each applicant. If an applicant refuses to sign the form, it shall be explained to him/her that the application cannot be processed. • All verification of employment shall be done via phone, internet or fax. • All employees are required to sign an Authorization for Release of Information authorizing Health Carousel to release copies of credentials and required documentation to client facilities for the purpose of assuring all contract requirements are met prior to placement. • When transmitting confidential information either by fax or email, the following procedure is to be utilized: o Call ahead to ensure that the intended recipient will pick up the fax. o Use cover sheet containing confidentiality statement o Confirm accuracy of destination fax number or email address prior to transmitting confidential documentsMaintaining Information SecuritySecurity is generally defined as having controls, counter-measures and procedures in place to ensure theappropriate protection of access to information, assets and valuable resources. Health Carousel has developedthe following procedures to maintain the security of such resources. • During normal business hours, documents containing confidential information shall not be left unattended where they would be visible to the casual observer. Confidential information documents will be locked in a cabinet if the office staff managing the documents must leave the work area unattended for any period of time. • During hours when the Health Carousel office is closed, all employee and client records are kept under lock and key • Employees shall change their computer program passwords according to Health Carousel’s Password Policy and any time they suspect that their passwords have been compromised • When not in use all workstations shall be locked or logged-off • Sensitive or confidential information shall be shredded, not left in the trash bin.Communicating and Enforcing the Policy • This policy shall be communicated to administrative employees at the time of hire or at any time when there is a change in policy. • The policy shall be made a part of orientation for new employees • Health Carousel’s management shall be responsible for enforcing the policy.POTENTIAL CONSEQUENCES OF VIOLATION OF CONFIDENTIALITY 51 OPERATIONS MANUAL | v13018

Any violation of confidentiality of information exposes Health Carousel to legal liability and jeopardized HealthCarousel’s professional reputation.The following outlines the procedure for responding to a breach of security involving personal information ineither paper or electronic format.Personal information is defined as: • Social Security number • Driver’s License, State Identification or Passport numbers • Direct Deposit information • Personal address & contact information, including home phone and cell phone numbers • Personal Health InformationPersonal information is considered to have been acquired or reasonable believed to have been acquired by anunauthorized person in any of the following situations: • Loss of documents: lost or stolen documents containing personal information • Loss of computing system: loss of any server, desktop, laptop or digital assistant containing unencrypted personal information • Hacking incident: a successful intrusion of computer containing employees’ personal information • Unauthorized data access: situations where unencrypted personal information has been downloaded, copied or used by an unauthorized personContainment: The first priority after a security breach is discovered is to notify the owner immediately.Classification: The Owner must determine the extent of the breach in terms of its extent and seriousness.Further action must be taken to contain the breach or recover missing data. If the breach involves theft ofelectronic equipment, the police will be notified.Notification to Victims: The Owner shall notify affected individuals without unreasonable delay. However,notification shall be delayed if law enforcement informs the Owners that disclosure of the breach wouldimpede a criminal investigation. Notification can be provided via written notification, electronic or notificationby phone provided that contact is made directly with the affected person.Contents of Notification: Notification shall be clear and conspicuous and include a description of the following: • The incident in general terms • The type of personal information that was subject to unauthorized access • Actions taken to prevent personal information from further unauthorized access in the future • A telephone number that the person may call for further information and assistanceThe following actions will be taken against perpetrators (i.e. anyone violating confidentiality policies): 52 OPERATIONS MANUAL | v13018

• Any employee who willfully breaches the confidentiality and security guidelines will be disciplined up to and including termination of employment, and if state or federal laws have been violated Health Carousel reserves the right to press criminal and/or civil charges, as applicable. • Any employee who accidentally breaches confidentiality guidelines will be subject to progressive discipline, dependent upon the seriousness of the breach: o A written reprimand for the first occurrence o A written reprimand, counseling and re-training on the policies regarding confidentiality of information for a second occurrence. o Termination for a third occurrence.Healthcare Professional Job Description – Case ManagerJob Description:Works as a local contract or traveling healthcare professional at a Health Carousel client facility, and inaccordance with the limitations and scope of care in accordance with the applicable state practice act.Coordinates with physicians, staff, and other healthcare providers regarding patient care. In conjunction withphysicians, develops treatment plan, monitors all clinical activities, makes recommendations for alternativelevels of care, identifies cost-effective protocols, and develops guidelines for care. Prepares for and assisting with surgical procedures- sets up thenecessary instruments- supplies- and equipment for operations- according to the routine of the surgeon.Knowledge of instruments and sterile technique in the O.R. Assist with duties as assigned. Surgicaltechnicians (aka, scrub or operating room technicians) assist in surgical operations under the supervision ofsurgeons, registered nurses or other surgical personnel. Primary responsibilities include preparing theoperating room, ascertaining that all equipment is working properly and preparing the patient for surgery.May also transport patients, observe vital signs, check charts and assist the surgical team in dressing for thesurgery.
