Personnel File (Trailborn)

FMIYLEPERSONNEL My employee file contains information required by law, as well as work and salary history, performance evaluations, benefit coverage, and information needed to identify my job. Access to my personnel file is restricted to authorized individuals within Trailborn. My medical information is considered extremely confidential and is maintained separately from non-medical personnel information. I have the right to inspect certain documents in my personnel file, as provided by law, in the presence of a representative of the Department of People & Culture, at a mutually convenient time. Access will be granted within 30 days of my written request. I may add comments to any disputed item in my file. I may also request a copy of my personnel file, even after my employment with Trailborn may have ended. I may be charged for the cost of copying personnel records. Trailborn will provide me or my authorized representative with access to and a copy of my personnel files in accordance with state law. My Personnel File 1
