4.7. Customer DocumentsThe Customer Document section/form assists SSL in identifying and verifying the identity ofits clients. Clients are required to upload ID documentation and other KYC Documents,including source of funds verification, proof of address etc. Non-Jamaican residents mustupload notarized documents. Acceptable notaries are: Justice of the Peace, Notary Publicand Licensed Attorneys-at-law. Required fields are marked with (*), and need to bepopulated before the form can be submitted. 51

4.7.1. Uploading a DocumentThe Customer Document section requires the client to upload KYC documents. Toaccomplish this click for each question as seen below to select the inputdocument to be uploaded.This will render a dialog box to select the file from your computer or mobile device. Acceptedfiles types are jpg, png, pdf, doc, docx and xls. Once the desired files have been selected and 53

It appears when the cursor is positioned over the icon on any form, as shownbelow:4.8.3. Close Embedded-formsEmbedded-forms are forms within other forms, for e.g. the PEP and FATCA forms which areembedded in the Account Opening Form.
