Review the Terms and Conditions and click “I Accept”

Enter your security token which was provided to you at sign-up and date of birth. If the patient has anemail address you can enter it here.

Otherwise click on the check box for “I do not have an email address” and it will them prompt you for alast name.

Complete the steps for enrollment including creating username and password, create a second loginsecurity question, and create a security question for password recovery. Click on “Submit” at the bottomwhen you are done.
