The objective of DCM is to streamline actions to be taken on drivers that commit violations/misconduct YARD INSPECTION 22 Jul 2020 : Workplace inspection was held at Swift Haulage Westport yard. ROAD SURVEILLANCE Jul 2020 : Road surveillance was conducted by Driver Trainer, Md Hairolazaman DRIVER COMPETENCY & TECHNICAL TRAINING 11 Aug 2020 : Driver Competency & Technical Training (DCTT) was held at SILS Training Room attended by seven (7) new drivers. Conducted by driver trainer, the training contents were Safe Driving Technique, Driver Responsibility, Fatigue Management, Emergency Response and Basic Fire Extinguisher TOOLBOX BRIEFING 10, 11, 12, 17, 21, 25 & 27 Aug 2020 : Toolbox briefings were done by Central Driver Trainers at Tanjong Sijangkang, Swift Haulage Northport & Westport, Tanjong Port Klang, Tanjong Bukit Beruntung and Trinity (subcon) GROUP HSEQ 4

DRUG 10, 12, 17, 21, 25 & 27 Aug 2020 : Drug test was conducted by Central Driver Trainers and TEST HSE reps at Tanjong Sijangkang, Swift Haulage Northport & Westport, Tanjong Port Klang and Tanjong Bukit Beruntung with a total of 24 drivers tested. All results were negative PM INSPECTION 10, 17, 21 & 27 Aug 2020 : PM inspection was carried out at Tanjong Sijangkang, Bukit Beruntung and Port Klang, led by Central Driver Trainers and assisted by HSE reps BEHAVIOUR OBSERVATION SYSTEM (BOS) 18 & 24 Aug 2020 : Behaviour Observation System (BOS) was conducted by Central Driver Trainers (Cikgu Azmi and Capt. Azatul and Mr. Ropathy to assess on SMC’s compliance with APAD’ & DOSH’ ICOP requirements ON-JOB – TRAINING (OJT) RHM & SHM 26 Aug 2020 : 5 Aug 2020 : An on-job-training by PCG personnel to Swift on Road RTOG training was Hazard Mapping & Site Hazard Mapping at Indaplast Material Resources conducted by Pn. HANDS & FINGERS INJURY PREVENTION CAMPAIGN 24, 25 & 27 Aug 2020 : A safety campaign was held at Port Klang & Pulau Indah warehouse and Q-Team Port Klang focussing on prevention of hands & fingers injury, calling all staff and general workers to be more cautious and to appreciate the value of having hands & fingers hence to ensure safety in their daily activities DRUG TEST 14, 24, 25 & 27 Aug 2020 : Drug test was conducted by Pn. Noormah at P. Klang & Pulau Indah warehouse and Q-Team P. Klang with a total of 26 staffs tested. All results were negative HEARING SIRIM17 Aug 2020 : Recertification audit by SIRIM for CONSERVA- MS224:2005 held at Q-Team Re-tread Plant AUDIT 7 Aug 2020 : HCP Program was held at Q-Team tyre workshop by PROCOMA, as to assess TION PROGRAM whether employees are exposed to excessive noise levels at the workplace as per OSH Act, 1994 under the OSH (Noise Regulation) 2019 (HCP) GROUP HSEQ 7

MySejahtera is an application developed by the Government of Malaysia to assist in managing the COVID-19 outbreaks in the country. It allows users to perform health self-assessment on themselves and their families.
