Fire-Rescue October 2018 Noteworthy Incidents 10/04/18 Medical Call – Squad 33 responded to Skanska worker injured from a fall. The patient was assisted across a narrow I-Beam to safety. Patient was transported to the hospital. 10/10/18 Station 33 – Station was manned for Hurricane Michael, which made landfall in Mexico Beach. 10/19/18 Mutual Aid to Pensacola Beach – Engine 33 was dispatched to a possible fire at the Holiday Inn Express on Pensacola Beach. Engine 33 personnel assisted with ventilation. 10/24/18 MVA – Squad 33 responded to a motorcycle vs. a semi- truck on Gulf Breeze Pkwy. The motorcycle operator was transported emergent to Baptist Hospital Pensacola. After the first ambulance departed the scene, the driver of the semi-truck suffered a panic-attack and was transported to the hospital non-emergent by a second ambulance. 07/17/18 Assist to PublicPSaegrev4ic9es – Engine 33 responded to a water main break on McAbee Court.

Fire-Rescue October 2018 Squad 33 Minor coolant leak, two different shops unable to find leak. Members checking cooling level on a more frequent Humvee 33 basis. Support 33 Out-of-Service, possible transmission issue. None Miscellaneous Santa Rosa County received a mutual request from State Emergency Response Team (SERT) to provide an engine strike team to Washington County. Five engines and one chief officer deployed for a period of five days. Assistant Chief Scott Kasper deployed with personnel from the City of Milton to staff one of the engines. Captain Jim Custred, who is a full-time firefighter with the City of Milton, and a part-time Code Enforcement Officer/Volunteer Firefighter with the City of Gulf Breeze, also deployed. Page 51

Public Services - Water & Sewer (SSRUS) October 2018 Monthly Highlights: WASTEWATER TREATMENT FACILITY Contractors were on-site to perform Geotech samples A total of 4.79 for the plant expansion. Staff installed a new float in inches of rain fell in October. the reject tank. The John Deere 575E tractor that was provided to the City from the FEMA ALT PROJECT required repair services that fell under warranty and therefore, no charges were made. The equipment was promptly repaired and returned. Staff prepared for Hurricane Michael. New carports were installed to cover and protect new tractors and various other pieces of vital equipment and material. Staff replaced two flights in the North thickener. WATER DISTRIBUTION & SEWER COLLECTION Staff repaired four water main breaks in October. The first three water main breaks were at West Madura Road, 1603 Bataan Lane and Bay Street, which were caused by aging pipes. The fourth main break was on Coronado Drive, which was caused by a contractor digging. Staff isolated the mains and made a repair. No critical use of facilities were affected. Samples were collected and returned in good standing. Staff identified and repaired several sources of inflow and infiltration (I&I) in the collection system by way of smoke testing and camera inspections. These activities are high priority for this year to reduce I&I into the collection system by at least 25%. Page 53

Public Services - Water & Sewer (SSRUS) October 2018 OPERATOR CERTIFICATIONS Service Worker I, Bryan Hix is working on the Water Distribution course work. The following Service Worker I's, Eric Dominguez, Cody Foster, Dillon Tyree, Will Berkley, Alan Durnil, and Charlie Laveck, all completed their course work for Water Distribution and are waiting to take the State test. Senior Service Worker, Brad Elliot, is working on his Water Plant Operator Certification course. Construction Foreman, Jeremy Norris, and Service Worker I, Steven Mowery, completed their Water Plant Operator Certification course work and are waiting to take the State test. Public Services - Water & Sewer (SSRUS) October 2018 Strategic Priorities Update: 1. OPERATOR TRAINING PROGRAM Staff will be completing a revised Operator Training Program to develop and retain the required certifications for operating the water, collection and treatment systems. The plan will include revised job descriptions to reflect relevant certification requirements for the various tasks in the field and regulatory requirements, and provide the pathway for employees to progress from Service Worker Trainee to Service Worker I, II, Senior and Foreman. 2. RECLAIMED IRRIGATION SUPPLY PLAN Staff is developing a reclaimed master plan of future reclaimed irrigation in conjunction with potential new developments. Completion expected in March of 2019. Public Services - Streets & Stormwater October 2018 Monthly Highlights: Staff and the inmate crew completed the routine activities of drainage and pump station inspections, roadway and right of way maintenance. Inmates are continuing to cut the City right of ways along with tree trimming and roadway asphalt repairs. Staff completed the 2019 Street Sign, Section 3 Audit. In addition, four signs have been identified to be replaced. Statistics/Performance Data: Page 57

Public Services - Streets & Stormwater October 2018 Strategic Priorities Update: 1. SIGNAGE EVALUATION/REPLACEMENT PROGRAM Staff developed a program for the evaluation and replacement of the street signage throughout the City. Implementation began in 2015, with an initial inspection and evaluation of each sign in the City, and then separating the City into 5 sections. A total of 709 sign locations were identified and those that did not meet the requirements outlined in the Manual for Uniform Traffic Control Devices (MUTCD) were repaired or replaced. City Staff personnel conduct annual inspections, focusing on one section per year. Signs that do not meet the MUTCD standards are repaired or replaced. When City Staff personnel identify signs in the field in the normal course of their workday that are out of compliance, a work order is created to address the issue. In addition, customers call in from time to time, reporting missing or broken signs, and City Staff personnel conduct reparations or replacements accordingly. A total of 297 signs have been replaced since the plan began. City Staff personnel completed the annual inspection on Section 1 in 2017, and completed Section 2 in 2018. 2. STORMWATER SYSTEM VIDEO INSPECTION Staff is developing a program to inspect, with a video record, all stormwater pipes in the City, every 5 years. The program will go out for bid in late 2018, with work to begin in FY 2019. Page 58

Staff continues work on leak survey, pressure checks, and valve maintenance. Staff made preparations for the impact of Hurricane Michael. Statistics/Performance Data: Strategic Priorities Update 1. SUPPLY MASTER PLAN Staff will review current purchase agreements and suppliers as well as develop any opportunity to purchase all or a percentage of gas supply at a lower cost (Henry Hub versus WCOG). The first formal draft was produced in April 2018, however, this will be reviewed annually after that point. Page 61
