We encourage you to carefully review the plans and select the right coverage for your unique needs.Who Is Eligible?As an active, full-time [, or part-time] employee working at least x hours perweek, you are eligible to participate in the [ABC] benefits program on [the first ofthe month following x days of employment] [the first of the month following yourdate of hire] [your date of hire].You may enroll your eligible dependents in many of the same plans you choosefor yourself. Proof of dependent status may be required to enroll. It’s important to review your benefit options andchoose the best coverage for you and your family.You have three opportunities to enroll or make changes: 1. Within 30 days of your eligibility date 2. COBRA Coverage – If You Leave Your JobYour [ABC] employer-sponsored benefits end on [your date of termination] [the last day of the month following yourtermination]. You may be eligible to continue coverage for certain benefits for yourself and your dependents as allowedunder the Consolidated Omnibus Budget Reconciliation Act (COBRA).The following benefits qualify for COBRA coverage: ƒƒ Medical and Prescription Drug ƒƒ Dental ƒƒ Vision ƒƒ [Flexible Spending Accounts (FSAs)] ƒƒ [Health Reimbursement Account (HRA)] ƒƒ Employee Assistance Program (EAP)If you participate in COBRA, you must enroll within 60 days of your notification date or coverage end date.
