INTRODUCTION History and Development of the GI Endoscopy Unit at the University of Malaya Medical Center The first gastroscope at University Hospital was bought in 1978 Professors Dr. Syed Noori and KL Goh, January 2016 The first fiber optic gastroscope was invented by Basil Hirschowitz and developed and introduced for use in patients by American Cystoscope Makers, Inc. (ACMI) in 1962. Since its introduction in the early 1960s, instru- ment companies, chiefly Japanese, have at breakneck speed come up with newer and amazingly well-con- structed scopes for clinical use by doctors. These compa- nies have continuously innovated and developed scopes of increasing sophistication, superior imaging as well as “ease of use”. The first gastroscopy in Malaysia was performed by Dr Peter Vaniasingham, Chief Surgeon of Penang General Hospital in 1973. Through the efforts of bodies such as the Japan International Cooperation Agency (JICA), gas- troscopes started to be used in a few hospitals in Malay- sia. At the University Hospital, Kuala Lumpur, a gastro- scope was first purchased and used by Dr John Stephens, an expatriate lecturer of the Department of Surgery in 1978. Very soon after that, Dr Christopher Loh Chi Loong returned from Gastroenterology Training in the UK and also started to use the gastroscope. He was followed by Dr Lee Hock Bee from the Department of Surgery and Dr Damian Wong from the Department of Medicine in 1979. Dr KL Goh had joined the team in 1984. There were few other endoscopists at that time, Drs KW Chang and SH Lee from Department of Surgery and subsequently Drs Pitre Anderson, KK Tan and Francis Yip from Department of Surgery in the late 1980s and early 1990s. Dr SY Chuah from Department of Medicine joined the unit in 1992 on his return from the UK. Gastroscopies and then later on, colonoscopies were originally carried out in the surgical minor operating theater (OT) in the surgical polyclinic located on the first floor of Menara Utama. This minor OT is still in existence up to the present day. In 1988, the endoscopy“unit”shift- Dr Damian Wong, a pioneer in GI Endoscopy in Malaysia ENDOSCOPY | 2016 9

Formed in 2005, it is chaired by Professor Dr Shuji Shimizu of Kyushu University and co-chaired by Professor Dr Ho-Seong Han from Seoul Na- tional University Bundang Hospital. University of Malaya (UM), as one of the oldest academic institution in the country, was one of the first academic centres that were linked with MYREN. The involvement of UM in the APAN MWG started since 2007 when the Gastroin- testinal (GI) Endoscopy unit of UM (represent- ed by late Associate Professor Dr Salem Omar) was invited to attend as remote participant in the MWG endoscopy teleconference held in conjunction with 24th APAN meeting in Xian, China. The network setup was done in MYREN Network Operating Centre in Cyberjaya, Ma- laysia. The live endoscopy procedures were performed in both Kyushu University and Kyoto 2nd Red Cross Hospital in Japan and were transmitted with 2-way communication to 6 other centres both in Asia and Europe. The highlight was the demonstration of en- doscopic submucosal dissection (ESD) tech- nique which is a novel endoscopic technique for treatment of early GI cancer. Participating centres benefitted greatly from the sharing of such technique via remote transmission. Professor Khean-Lee Goh Head, GI Endoscopy Unit c/o Department of Medicine Faculty of Medicine University of Malaya 50603 Kuala Lumpur Malaysia June 12, 2008 Dear Professor Goh It gives me great pleasure to confirm that the Governing Council of the World Organisation for Digestive Endoscopy has unanimously recommended that we designate your department as an OMED Center of Excellence. The OMED Centers of Excellence are restricted to a small number of endoscopy units that are chosen from around the world for the quality of their endoscopy service, the expertise of their faculty, but more particularly for the contribution they have made to international teaching in digestive endoscopy. The World Organisation recognizes that your department in Kuala Lumpur provides not only endoscopy of an exceptional quality, but in addition you have, over many years, established a regular international live endoscopy course employing a wide range of internationally renowned endoscopists. You are one of 16 Centers of Excellence and you or a colleague of yours will be welcome to attend the Annual Meeting of Centers of Excellence which we hold at the time of the American DDW. This meeting will enable you to meet with the other directors of the centers, who offer advice to the World Organisation of Digestive Endoscopy as to how high quality endoscopy can be promoted throughout the world, the way in which the World Organisation will be able to help, encourage and promote your own department and to consider ways in which the centers can work together to undertake research and teaching. I have great pleasure in welcoming you to the community. Yours sincerely Athony T. Axon OMED President 46 2016 | ENDOSCOPY
