Sign Solutions Return to Work Form Sign Solutions conducts return to work meetings when there is concern over attendance levels. Our aim is to ensure the health and wellbeing of our staff. We conduct return to work meetings to ascertain if there are any measures we can adapt to improve the health and wellbeing of the employee. As a general guide Sign Solutions uses the following as indicators of concern. In the case of a full-time staff member; - i) 10 working days over two or more period of absence in a twelve-month period ii) 4 separate spells of absence in a twelve-month period. If the Managing Director becomes concerned that an employee takes more sickness absence than would normally be expected, they will arrange a ‘Return to work interview’ with the employee and themselves, or the departmental head. This may subsequently lead to a verbal warning under the Disciplinary Procedure and could lead to the termination of the employee’s contract should there be no improvement. The Managing Director could ask the employee for permission to approach their GP or medical consultant in connection with sickness absence management. The Managing Director may also consider withdrawal of self- certification for future periods of absence. __________________________________________________________________________________ Employee Name;____ __ Managers Name;__ Date of meeting; _2__________________ Recent Dates and reasons for absence;-- Return date Days of absence Reason for absence Episode of Absence Date of absence Employees Comments;- Managers comments and any action points;- Any attachments such as FIT note etc V0.1 20/09/21
