Recertification Tracking Form 1. This tracking form will help you to fix all credits earned for OCI recertification. 2. This tracking form is intended for personal use and should not be provided for recertification. Educational programs № Event Date of the Number event of credits 1 2 3 4 5 6 RTF Total If you provide personal trainer educational courses, it can be counted as credits. Education courses must be at the high school/university level (regional, state, or national). You can earn no more than 30 % of required number of credits in this category and 1 credit for 1 hour. For recertification you should: • Have the required number of credits in compliance with the qualification requirements • Fill the recertification form • Have the current CPR certificate • Submit recertification form, previous certificate and current CPR certificate to the e-mail [email protected].
