STEP 1The employee’s workplace completes the requireddetails including their company Purchase Ordernumber on the “R&A” form which the employee thentakes along with them to their appointment with one ofEyres licenced Optometrists.It’s recommended the employee takes their existingprescription if they have one to the appointment.STEP 2 The PPE Supplier has a contract with their customer for the supply of EyresThe Optometrist completes theconsultation, advising the employee of prescription safety eyewear.the most suitable Eyres frame from the PPE provides their customer with theapproved package & completes the order ‘Request and Authorisation’ form along with adetails on the “R&A” form. list of selected Eyres licenced Optometrists.The form is then forwarded to the PPE Please note that the “Request andstore to be issued with a Purchase Order Authorisation’ form must be completednumber and returned so that theOptometrist who can then input the order in full for the Optometrist to be ableonline. to input the order online. STEP 3 Once the Optometrist receives the fitted prescription safety job from Eyres, they will advise the employee that their glasses are ready for collection. The Optometrist is to ensure that the employee signs & completes the collection details on the “R&A” form as confirmation that they are satisfied the frame has been supplied as ordered and is optically correct. The Optometrist then forwards the completed form to the PPE store and Eyres for invoicing to be finalised.

] PRESCRIPTION SAFETY PAPER TRAIL Contracted customer issues company PO to PPE store which is recorded on the ‘Request and Authorisation’ form. PPE store issues a PO and enters it onto the R&A form once they have received the script details from the Optometrist. Optometrist faxes or emails completed 'Request and Authorisation' form to PPE once the employee has picked up their prescription job and signed for them. Optometrist faxes or emails signed and completed ‘Request and Authorisation’ form to Shamir OHS along with a tax invoice for their dispensing fee. Shamir OHS invoices the PPE store the agreed wholesale price plus $12.00 +GST freight. Shamir OHS pays the Optometrist their dispensing fee within 30 days of receiving the completed ‘R&A’ form.PPE invoices the contracted company(the RRP pricing) once receiving thecompleted and signed ‘R&A’ form off theOptometrist.

Most of these injuries are minor but some accidents can result in serious injury, vision loss, blindnessor loss of an eye.Occupational health and safety awareness is growing considerably with industries. Organisations are nowintroducing OH&S polices into their procedures to ensure safe working conditions for their workplace.Attention to your working environment is vital to help avoid injuries. Wearing appropriate eye protectors andusing preventative measures can significantly reduce the risk of injury.Remember safety eyewear should be replacing every 12 months or after any impact. AUSTRALIAN STANDARDS FOR OCCUPATIONAL EYE PROTECTION (SAI GLOBAL) According to the Australian Commonwealth Work and Health Safety Act 2011, employers must, amongst other things, \"provide suitable protective clothing and equipment to staff\". “The person conducting a business or undertaking (PCBU) who directs the carrying out of work must provide the personal protective equipment to workers at the workplace, unless the personal protective equipment has been provided by another person conducting a business or undertaking”. This includes safety prescription eyewear. To be compliant with this requirement, protective prescription eyewear must comply with the AS/NZS 1337.6 standard. EYRES safety frames are manufactured to standards exceeding those of regular ophthalmic frames. Each frame model in the EYRES range meets the Australian / New Zealand requirements of AS/NZS 1337.6 & AS/NZS 1337.1. Shamir OHS is licensed to manufacture EYRES prescription safety eyewear under SAI Licence # SMKH21598. AS/NZS 1337.6:2012 Personal eye protection - Prescription eye protectors against low and medium impact. This Standard specifies minimum requirements for eye protectors fitted with prescription lenses intended to provide low or medium impact eye protection from flying particles and fragments in occupational situations. It does not apply to eye protectors in which the prescription lenses are fitted behind Plano protective lenses. These are covered by AS/NZS 1337.1. Prescription eye protectors complying with this Standard are intended to give both impact protection and prescribed refractive correction of vision. For protection from hazards other than low or medium impact, appropriate protection as recommended in AS/NZS 1336 should also be worn covering the prescription eyewear, e.g. dust, splashing materials, molten materials, harmful gases, vapours or aerosols, high energy flying particles or fragments. These are covered in AS/NZS 1337.1. Requirements for protection from sources of optical radiation, other than the sun, are covered in AS/NZS 1337, Parts 1 to 3. The Australian and New Zealand standard AS/NZS 1337.6 Prescription Eye Protection against low and medium impact specifies minimal requirement for the appliance to withstand the impact of a 6.35mm steel ball at a velocity of 40mps.
