How will You Pay for Long-term Care? Written By: Lori Givens – Wheat-Givens Financial Current studies indicate some form of Long-Term Care (LTC) will be inevitable for 70% of Americans. The question is, then, how will you pay for that care? Check out these strategies to help you decide if you’ll have the resources you need to be able to choose your care. Self-pay – Those who have saved diligently, been the beneficiary of an inheritance and/or have significant equity in a home they are willing to sell may find they have ample resources to self-pay for care. Careful, conservative analysis will help you decide if your resources can withstand what the future may bring. LTC Insurance – more and more people are choosing to increase the chances of not being a burden to their loved ones by choosing to invest in Long-Term Care insurance. These policies provide, essentially, a “bucket” of money to use for home health care that allows the recipient to remain in their home all the way through skilled nursing or memory care. While the “bucket” isn’t endless, it can be a lifesaver, especially for married couples who potentially need to pay for care for two people – maybe at the same time. Annuity conversion – If you are holding a non-qualified Annuity that won’t be needed for its guaranteed income benefit, look at a tax-free conversion (known as a 1035 exchange) to a Long-Term Care policy. This will allow you to LorweimthdraIw pfunsdus mtax free and make good use of an otherwise unneeded asset. Medicaid – Medicare does not provide LTC benefits. For Texans who can demonstrate that they have less than $2,000 (single) / $3000 (married) in assets (not including a residence of up to $585,000 in value) and no more than $2,313 (single)/$4,326 (married) per month in income, Medicaid may be available to provide skilled care, if the care needed is “medically necessary”. You may be surprised to find out that frequent falls or dementia may not qualify as “medically necessary”. Family and friends – most people will tell you they never want to be a burden to their children; however, those with limited resources may find themselves in need of care with no way to pay for it. Family and friends, coupled with occasional respite care for the caregivers, may be the only solution for getting care for those who couldn’t or didn’t plan ahead. It’s a good idea to meet with a Certified Financial Planner™ professional to evaluate your situation and plan the best course of action for your future. What are Activities of Daily Living (ADLs) and Why Are They Important? Written By: Cindy Arledge - Legacy Financial Planning Activities of Daily Living, or ADLs are comprised of six skills needed to live independently. Medicare/Medicaid, health care professionals, and insurance companies make decisions and provide services based on your ability, or inability, to perform these skills. ADLs include the ability to (1) feed yourself, (2) walk and transfer from sitting to standing, (3) dress yourself, (4) control your bladder and bowels, (5) use the toilet without help, and (6) bathe yourself. Instrumental ADLs, or IADLs, refer to more complicated skills that include (1) paying bills and managing finances, (2) driving and arranging transportation, (3) shopping for items needed for daily living, (4) planning, cooking and preparing meals, (5) cleaning and maintaining a home, (6) communicating by phone or mail, and (7) obtaining medications and following directions. Having difficulty preforming these activities may be an indicator of dementia, or other health issues. Seniors who accept help with IALDs typically remain healthy and independent longer. Advanced Activities of Daily Living, or AADLs, are three categories of activities that include intellectual, social and physical. Challenges with ordinary activities such as hobbies, travel, or exercise are often the first signs of dementia and indicators that it is time see your health care professional or change your lifestyle. The natural process of aging is a progression of decreasing skills. Recognizing AADLs, IADLS and ADLs as indicators of the aging process provide opportunities to make changes to retain quality of life. Caregiver Fatigue in the Heart of Senior Care By Dannelle LeBlanc, Certified Senior Advisor and Caregiving Consultant “After 8 years, I never imagined my life would still be on hold,” says a client caring for her 94 year old mom with dementia. “I don’t know how I can keep going.” Many of the 40+ million unpaid family caregivers in the U.S. can relate to her struggle. Balancing care for someone in vulnerable health with your own wellbeing is a huge challenge. Family caregivers serve as the backbone of our long-term health care system, providing 80% of care, estimated at $470 billion nationally (2015 AARP Public Policy report). Care responsibilities can begin due to a medical crisis, or a gradual, intensifying need for care. In either case, it’s understandable how caregivers can lose sight of their own needs when focused on another’s. “Caregiver fatigue” is a general term for the physical and emotional exhaustion caregivers frequently experience. Signs may include withdrawal, hopelessness, changes in sleep cycle, irritability, and both physical and emotional depletion. Without coping strategies, the level of severity can escalate. Another aspect of caregiver fatigue is “compassion fatigue”, in which the ability to empathize with and provide care for a loved one is compromised due to extreme tension and involvement with another’s suffering. Coping strategies that work for an individual caregiver’s situation are key to maintaining wellbeing. As a senior care community, we can help by redefining senior care to include resources specific to the needs of family caregivers. Other ways to consider managing caregiver fatigue include: • Awareness of changes in stress levels. Keeping a journal is a great way to stay in touch with gradual changes in your physical and emotional condition. • Defining what self-care means in your caregiving situation. The reality of providing ongoing care means that self-care looks different than it did before. Even 10 minutes a day to meditate, stretch, or listen to your favorite music can encourage your wellbeing. • Asking for specific kinds of help when possible. There are times when our loved one may not want help, but you can ask for help for you. Who can you ask for help with household chores, stopping by to give you a break for an hour, or running errands? Be specific in your requests for help. • Finding a support network that works for you. It makes a difference when you connect with others who get it. Types of Senior Living Options Written By Olia Davis – Oasis Senior Advisors INDEPENDENT LIVING Designed for older adults who can live on their own but are looking to enjoy the benefits of a community that offers residents companionship and recreational activities in a secured environment. Services such as housekeeping, meals, laundry and transportation may or may not be included, but are typically available. ASSISTED LIVING Assisted living communities offer a safe, caring environment for those individuals who are beginning to experience difficulties living on their own. They provide a special combination of housing, personalized supportive services and health care designed to meet the needs of those who don’t require full-time skilled nursing care but might need help with activities of daily living. They generally bridge the gap between living independently and living in a skilled nursing facility. RESIDENTIAL CARE HOMES Residential care homes offer long-term, in-home care for seniors who need moderate assistance with daily activities. Services are either live-in or around-the-clock. The homes are smaller in size and usually accommodate between 2-8 residents. Residents have a private or semi-private bedroom but shared bathrooms. MEMORY CARE This specialized senior living option caters to a safe and secure environment for those suffering from illnesses such as dementia and Alzheimer’s. Programs and activities are planned with dementia care in mind. Offering around-the-clock care, residents receive the attention they need. CONTINUING CARE RETIREMENT COMMUNITIES (CCRCS) This option offers housing packages that allow individuals access to numerous senior housing options, including independent living, assisted living, skilled nursing facilities, and more. This option is ideal for those who may need to adjust their living situation over time, as it is easy to transfer to another housing type. SKILLED NURSING Skilled nursing facilities are unique in that they provide 24-hour medical care.
