Dear Student, We are delighted to welcome you to Amlotus English Language School! Amlotus was founded in 2010, and has been in continuous operation since that date. During this time, we have had many students enrolled in our programs. Our former students have reported that they were extremely satisfied with our services and that the level of proficiency attained through our English Language Program has served them well in their subsequent careers. At Amlotus, our faculty members have excellent credentials. Our instructors, most of whom have been teaching for ten to twenty years, are experts in their field. Their goal is to assist you in the successful completion of the program, thereby enabling you to advance in your career and ultimately attain your dream. Amlotus provides you with an opportunity to learn English from gifted teachers in a nurturing, intimate and supportive setting among classmates who may become some of your lifelong friends. 1. MISSION STATEMENT Amlotus, LLC, is committed to providing high-quality English instruction in the form of ESL, TOEFL classes, and tutoring in Language Arts. We strive to assist students in their academic, cultural, linguistic and professional development so that they will be able to be successful in their endeavors. We provide a supportive and positive work environment in order to attract and retain teachers and support staff of the highest caliber. We encourage the use of a variety of teaching styles, modern technology, and experiences outside of the classroom to meet the needs of students with different learning styles from different backgrounds. We seek to provide an enriching, professional, and comfortable environment for our students to learn and socialize in, and we offer a plethora of student services to assist our students with any issues they may encounter, whether those concerns are of an academic or personal nature. . 2. UNDERSTANDING IMMIGRATION FORMS (Applicable for F-1 International Student Only) FORM I-20 All F students who study in the United States need a Form I-20, “Certificate of Eligibility for Nonimmigrant Student Status.” This document is your permission to stay in the United States as a full-time student. If your program of study takes longer than the time listed on your I-20, you must contact your International Student Advisor (DSO: Designated School Official) at least 30 days in advance to extend your stay. This will require updated financial documents. Staying beyond the end date listed on your I-20 is a violation of your status. Failure to apply in a timely manner for extension of your I-20 or a program is considered a violation of status, which will cause a loss of your F1 international student status. You are eligible to extend your F-1 status for an academic reason and medical reason. PASSPORT Your passport must be valid during your stay in the United State. In the event your passport is close to expiring, you must apply for an extension through your embassy at least six months prior to the expiration date. Pursuant to the USCIS immigration law, an expired passport is a violation of status even if you have attended school. STUDENT VISA An F1 student visa is a nonimmigrant visa for people who intend to study full time in the United States, which is placed inside your passport. Your visa gives you a permission to ENTER the United States, which is different from Form I-20 that is your permission to STAY in the United States. As long as students have a valid I-20, they are allowed to stay in the United States with an expired visa. If you plan on traveling outside of the U.S. with an expired visa, you must renew your F-1 student visa at the U.S. embassy in your home country to re-enter the U.S. I-94 Form I-94 Form is the arrival and departure record in either paper or electronic format, issued by a Customs and Border Protection (CBP) officer to foreign visitors entering the U.S. The I-94 Form indicates your F-1 visa status and your admission to the U.S. for Duration of Status (D/S). Duration of Status is indicated by the dates on your I-20. An immigration officer will take your I- 94 card attached in your passport or update your departure in the electronic record when you leave the U.S. WHAT IS YOUR STATUS F-1 is your status in the U.S. As an F-1 nonimmigrant student, you are “in status” if you follow all of the rules and regulations. If you become “out of status,” you must speak with your international student advisor (DSO) immediately. In the case of “out of status,” you will need to apply for ‘reinstatement’ or leave the United States immediately. WHAT IS A F-1 STUDENT AMLOTUS, LLC STUDENT HANDBOOK PAGE 6

An F-1 student is a person who comes to the U.S. and pursues a full-time academic or language program. F-1 students who are admitted to Amlotus must maintain a full course of study at least 18 hours per week. Failure to do so will result in a loss of your F-1 student status. 3. ADMISSION REQUIREMENTS & PROCEDURES FOR STUDENTS a. Admission Requirements FOR ALL STUDENTS To be enrolled in Amlotus Program, Applicants must meet all of the following criteria: IEP (Intensive English Program) 1. AGE – Applicant must be at least 17 years of age by the time he/she starts school. 2. HSD or GED/TASC– A copy of High School Diploma or Transcripts that indicate a high school graduation date; College Diploma or Transcripts; OR GED (General Education Diploma) /TASC (If the document is not in English, it must be translated into English.) * Students who enroll for one-on-one English tutoring are not required to have a HSD or GED/TASC. b. Affidavit of Support Form completed and signed OR Notarized Letters from parents and/or sponsor(s) from each financial provider which stats that they will be the financial providers for the student’s studies at Amlotus, LLC. 2. Bank Certified letter(s) or Statement showing enough funds (*) to cover the student’s financial needs for his/her duration of study at Amlotus including tuition, books and supplies, and living expenses. It must be valid for at least 3 months from the date of registration. (* Estimate of expenses for students enrolled in Amlotus Intensive ESL Programs for an academic year is $15,000.00 US dollars. If you have a dependent(s), the amount of $7,500.00 US dollars for a spouse, and (or) the amount of $5,000.00 US dollars for each child must be added respectively.) 3. Valid Passport – It must be valid for at least 6 months from the date of expiration. 4. International Student Application for SEVIS I-20 must be completed with applicant’s signature. 5. US $50.00 payment for the application fee. (Application fee is non-refundable.) c. d. Admission Procedures Amlotus requires all prospective students to complete the formal admissions procedures: 1. Prospective Students must fill out an Amlotus application form- either International Student Application for SEVIS I-20 form or Common Application for Non-I-20 students. 2. Students must submit all documents prior to an acceptance decision being made. 3. For overseas students, prospective students will receive Pre-arrival Package by mail or e- mail. 4. Students are required to take a placement test in order to determine placement into the appropriate level. 5. Upon the completion of the placement test, students can register for the course. Student needs to sign an Enrollment Agreement and any other contractual documents with Amlotus and pay for tuition and registration fees before entering the class. 6. Students must attend an Amlotus orientation prior to attending their first class. If the student misses the regularly scheduled orientation, then he/she must arrange an orientation with the administration before he/she is allowed to attend classes at Amlotus. 7. During the orientation, students will receive Amlotus Catalog, Student Handbook, and curriculum/ course schedule information and complete the orientation checklist with students’ attestation. 8. Students must read the Amlotus Catalog and the Student Handbook information to understand all details related to the courses and admissions and sign an acknowledgement that they have read and understand the policies. 9. Student must meet All admissions requirements, including completion of student orientation, the signing and returning of important forms, and all financial obligations. 10. If a student has questions regarding Amlotus’ policies, regulations, admissions procedures, curriculum/ courses to choose, attendance policies, satisfactory academic progress, and any personal and immigration matters etc., he/she may consult with a School Agent or Staff Member in person at school, by phone 212-912-0100, e-mail at [email protected], or appointment, to ask for advice. e. Placement Procedures and Instruments: Amlotus uses the CaMLA (Cambridge Michigan Language Assessment) EPT test for Listening/Grammar/Reading/Vocabulary, the CaMLA speaking test, and the CaMLA writing test to determine initial placement. School agents contact the students AT LEAST a week prior to classes starting to make arrangements to take the placement test. When students at Amlotus are initially tested, either the Academic Director or Academic Coordinator administers the tests. After the CaMLA EPT test and CaMLA writing tests, the CaMLA speaking test is administered. Once the tests have been evaluated, the student will meet with the school agents to discuss the results of the test and the student’s initial placement into an ESL class. Any student who feels that the result of the test is inaccurate or not representative of their abilities can appeal the placement or progression by notifying the Academic Coordinator/ the administration. In writing, students can compose a work- related email/letter expressing thanks in detail, a short, concise formal report explaining how a product works, and provide answers to open-ended questions in a survey about familiar products or services. • Can understand written feedback on job performance, and a complex form for company-offered services (e.g. medical insurance). • Can identify relevant articles and reports on a range of professional topics, a list of linguistically complex written questions to ask during a job interview, distinguish between fact and opinion in complex business contexts, and comprehend linguistically complex texts in their field. • Can give a short, prepared update on a work-related project, and suggest a resolution to a conflict in a simple negotiation using fixed expressions. • Can write a cover letter addressing specific information mentioned in a job posting, respond effectively to emails requesting work-related information, and use appropriate register in work-related written communication. Reference – CEFR (Common European Framework of Reference for Language) The global scale of CEFR is issued by the Council of Europe and used around the world to describe an international standard in language teaching. <Global Scale of CEFR> AMLOTUS, LLC STUDENT HANDBOOK PAGE 13

issued by the Council of Europe Reference – GSE (Global Scale of English by Pearson) GSE extends the CEFR by pinpointing on a scale 10 to 90 what needs to be mastered for the four skills of reading, listening, speaking and writing within a CEFR level. New Students are allowed to register for the course by the end of week 4 after the course starts. Amlotus considers closely the exact placement of the student. We look at the raw number, and determine if the student is at the low, mid or high end of the placement. If a student is at the low end, and it is week three or four, they are generally placed down a level as to not interrupt the existing class and where the class is in the syllabus. New students are assessed by their teachers as well to determine if placement is accurate through a diagnostic test. These tests take into consideration the material the student may have missed, once again, ensuring that the flow of the course is not interrupted. g. Changes to Initial Placement For English language training program Any student or an instructor who feels that the result of the initial placement is inaccurate or not representative of the student’ abilities can appeal the placement or progression by submitting a request form for re-evaluation of placement to the Academic Director/ the administration. Upon the submission of the request form, the student may be allowed to retake the placement test, at the discretion of the Academic Director, or another member of the administration. Upon completion of the placement test, the Academic Director confers with the classroom teacher, and finalizes placement. Within 48 hours from the test being taken, the Academic Director meets with the student and the instructor to review both the initial and secondary placement test results. The joint decision of the Academic Director and the classroom teacher is final and cannot be appealed. The final decision letter is sent to the student and also placed in the student’s folder. If the School does not accept an applicant, all fees paid to the Amlotus are refunded except non- refundable course registration/ application fees. The School reserves the right to reject a student previously accepted if the items listed above are not successfully completed. h. Appeal to course grades or promotion decisions Students also have the right to appeal course grades and promotion decisions. The first step of the appeal process is for the student to speak with the instructor who assigned the grade. If the student is not satisfied after speaking with the instructor, he/she can make a formal appeal to the grade or promotion decision by submitting a request form for re-evaluation of course grades or promotion decisions to the Academic Director/ the administration. Within 48 hours from the submission of the request form, the Academic Director will meet with the student and the instructor to review the student’s grade and progress. The joint decision of the Academic Director and the classroom teacher is final and cannot be appealed. The final decision letter will be sent to a student and also placed in the student’s folder. i. Appeal to determination of successful completion of the program Any student who feels that the result of the determination of successful completion of the program is inaccurate can appeal the determination by notifying the Academic Director/the administration in writing via an email or mail. The student will make arrangements to meet with AMLOTUS, LLC STUDENT HANDBOOK PAGE 15

the Academic Director. The Academic Director will meet with the student and the instructor to review the components of the determination. The decision of the Academic Director is final and cannot be appealed. The final decision letter will be sent to a student via an email or mail and also placed in the student’s folder. 5. PROGRAMMATIC GOALS Upon completion of the Amlotus Intensive ESL Program, students will · Demonstrate ability to comprehend, interpret, and apply spoken and written forms of communication. · Demonstrate awareness of both explicit and implied meaning in extended spoken discourse. · Read with comprehension of both content and organization. · Recognize and utilize key vocabulary appropriate to a variety of situations. · Apply a range of language structures to interact effectively. · Write about a variety of topics with level-appropriate correctness, clarity, and detail. 6. STANDARDS OF CONDUCT a. Student Contact Information 1. ALL students must provide their current address, telephone number, and e-mail address to the school upon registration. 2. Any changes to the contact information must be reported to an Office manager immediately, but no later than three (3) business days after the change takes effect. Students should contact an Office manager if they have any questions. 3. If a student is going to be on a leave or away from school for an extended period of time, he/she must provide Amlotus with a number through which they can be contacted while they are away. b. Standards of Behavior Students must follow all policies, regulations, and instructions from school staff and school management. 1. Act politely and professionally at all times. 2. Smoking is strictly prohibited on school premises. 3. Turn off cellular phone when class starts. 4. Keep school neat and clean – no spitting or littering on school premises AMLOTUS, LLC STUDENT HANDBOOK PAGE 16

5. Attend every class on time and complete all assigned course work, course activities, and required quizzes and exams. 6. Bring course textbooks to class at all times. 7. Submit your own work. Plagiarism (copying another student’s work) is strictly forbidden. 8. Only use the existing software installed on the school’s computers. Students are not permitted to install software on any school computer or make any changes to computer systems. 9. You are responsible for any damage to computer equipment, furniture, or other school property. In the event that damage occurs, reimbursement will be sought from the student. Immediate Expulsion 10. Student may be expelled immediately for the following incidents: stealing; seriously interfering with normal school operations; abusive behavior to school staff or other students; possessing a weapon; threatening the school staff; coming to school under the influence of alcohol; threatening or harming other students; or any similar behavior. 11. It is prohibited to possess, use, or distribute drugs or alcohol on school premises. Such behavior may be cause for immediate suspension or termination by a member of the Administrative Team c. Suspension / Dismissal (DM) Amlotus management reserves the right to suspend or dismiss a student for any conduct which seriously violates the Standards of Student Conduct. Prior to suspension or dismissal, the Amlotus Administration shall give the student oral or written warning. If the student has no intention of correcting his/her behavior, a formal suspension or dismissal might be issued. Under emergency circumstances, the President, Academic Coordinator, or the office manager has the right to immediately and temporarily suspend a student for seriously and dangerously obstructing or interfering with school’s normal operations. Further legal action might be taken against the student’s inappropriate behavior in order to protect Amlotus as an educational institution. If a student can correct his/her behavior, the “Suspension” may be removed and the student will be able to continue his/her studies at Amlotus. However, if a “Dismissal” decision is made by the school management, the student will be dismissed immediately from the school. If a student has been dismissed, he/she may re-apply for admission after a period of 90 days. The student must meet with the Academic Director and Academic Coordinator, and, if re-admitted, student will be placed on a behavior/academic contract for the first 30 days of his/her return. If the student breaches that contract, the contract may be extended, or the student may be suspended or dismissed again. If the student fulfills the obligation of the contract, the contract will end, and the student will resume regular study. AMLOTUS, LLC STUDENT HANDBOOK PAGE 17

7. ACADEMIC POLICIES & REGULATIONS a. Attendance (P) 1. All students must maintain an attendance rate of at least 85% of classroom instructional hours for each course in order to make a normal progress to the next course. Additionally, F1 visa holders who fall below the required 80% attendance requirement, will be reported to the US Department of Homeland Security by Designated School Officials (DSOs) and his/her enrollment and statues shall be immediately terminated by School. 2. F1 international students must maintain full-time student statues in compliance with Federal rules by attending class for a minimum of 18 hours of classroom instruction each week. 3. Any absences including tardiness up to 15% of instructional hours do not need to be made up, but any absences including tardiness from 16% of instructional hours must be made up to achieve academic progress. Failure to do so results in termination. Students are only allowed to have make-up classes up to 15% of instructional hours. Attendance Warning Procedures · 1st Warning – If a student who misses 15% of any course hours will receive an attendance warning letter and be informed to improve his/her attendance rate. · 2nd Warning – If a student’s attendance rate still falls below 85% within 2 weeks after the first warning letter, the student will receive the 2nd warning letter. This time the student is asked to meet with the administration team to discuss how to improve his/her attendance. · Attendance Probation– After 2nd warning letter, if the student’s attendance rate does not improve up to at least 85% within another 2 weeks, the student will be placed on attendance probation. If the student fails to maintain the required attendance rate during the attendance probation, he/she will be dismissed. Even if attendance is not a part of the grade, it is a critical of language learning, so Amlotus encourages students to attend all classes and complete all assignments. · Final Termination Letter- For F-1 international students, if the student’s attendance rate falls below 80%, the student will be dismissed immediately and the termination letter will be sent to the student via an email or by mail and place in the student’s folder. In this case, his/her attendance information will be reported to the US Department of Homeland Security by Designated School Officials (International Student Advisor), and his/her F1 statues shall be immediately terminated by School. While attendance and homework are not a part of the grade, they are a critical part of language learning. To ensure that you acquire the needed English skills, it is best to attend all classes and complete all assignments. AMLOTUS, LLC STUDENT HANDBOOK PAGE 18

b. Tardiness (L) 1. If a student arrives in a classroom more than 15 minutes after the class starts, he/she will be marked as “Late” (L). The teacher will speak with the student. Excessive tardiness will not be accepted. Any student who misses more than 25% of a class lecture will automatically be marked absent for the rest of the class. While the student is free to stay in the class, he/she will not be marked present. 2. For attendance purposes, three tardies will constitute one absence. 3. Non-Attendance and lateness will not affect a student’s financial obligations to Amlotus. 4. If a student is persistently tardy every week, he/she will be referred by the instructor to a member of the Administrative Team. It is a student’s responsibility to inform Amlotus if he/she will miss or has missed a class. The student must notify the instructor or a school agent as soon as possible, by phone or by email. Early Departure: If a student leaves the class 15 minutes or more prior to the class being dismissed, the student’ attendance on that day will be marked as absence. c. Absences (A) and Make-ups (M) If a student wishes to have an absence excused, the student must provide written documentation detailing the reason for the absence. 1. An excused absence is one where the student has submitted to an administrator the appropriate documentation and a member of the Administrative Team approves the excused absence. If an absence is excused, the student will be permitted to make up any missed work for full credit. 1. An unexcused absence means that the student was absent and provided no documentation to the school explaining the absence. The student will be allowed to make up any missed work. The instructor, however, reserves the right to issue partial credit. 2. Excused or unexcused absences are not allowable absences. It remains the responsibility of the student to make up the missed course work to meet attendance requirements for graduation. Class Make-up General Procedures For missed classes: A student needs to make up the missed class by working with the instructor during his/her office hours or taking a make-up class School may arrange. Any work missed due to an excused or unexcused absence shall be completed within five (5) class AMLOTUS, LLC STUDENT HANDBOOK PAGE 19

days from the date of return. If the student wishes to take more than five (5) days, he/she must make arrangement with the next available instructor and a new arrangement form with a next make-up date(s) shall be submitted to the administration. Any students who need to make up work must complete the following steps; · The student should fill in the Course Make-Up Form to arrange specific days and times in which the missed course work will be made up. · The Course Make-Up Form must be signed by the makeup instructor to verify the student’s work is completed and also signed by School Director for approval. · The student is responsible for returning the makeup sheet to the classroom instructor and following up to make sure the instructor has graded. · For make-up classes, students will be charged for make-up classes. For missed Quizzes, Exams, or Projects: A student needs to complete the Quiz, Exam, or Project as soon as possible. The instructor will grade the Quiz, Exam, or Project. In the case of an unexcused absence, the grade may be one level lower than the normal grad (ex. C- instead of C). If a student has an excused absence, the instructor must allow the student to make up the test, quiz, exam or project to be made up for full credit. Once a student has completed all work missed, the instructor will grade the work and assign the student a grade within 72 hours of the makeup work being received. Types of Makeup Class (1) Late Register Makeup Class: Any students who register for the course between the first week and the 4th week of class should make up 85% of the missing hours. Makeup class will be held on Saturdays. Late register make-up classes are free of charge. (2) Low Attendance Makeup Class: a. Any students whose attendance rate falls below 85% should make up. On weekly basis, the students with below 85% attendance rate will be notified of their makeup class status. • For 216 hour course, a student is asked to make up the class if he/she misses more than 32 hours. • For 240 hour course, a student is asked to make up the class if he/she misses more than 36 hours. b. Should the student need to make up in this category, he will fill out Course Makeup Form with school agent. Makeup class will be held on Saturdays. (3) Holiday Makeup Class In order to make up the missing class hours due to holidays or emergencies such as bad weather conditions, students will be notified of their holiday/emergency makeup classes on the first day of class. The makeup class will be held on Saturdays. Holiday/emergency makeup classes are free of charge. AMLOTUS, LLC STUDENT HANDBOOK PAGE 20

d. Leaves of Absence (LOA) If a student has circumstances which will necessitate them being out of school for an extended period of time, he/she should request a Leave of Absence (LOA). A leave of absence means that the student will be out of school for a period of time, but has plans to return by an agreed upon on date. Any types of leaves of absence herein must not exceed five (5) months in the aggregate for that student’s course of study. Any leaves of absence must be approved by the Director PRIOR to a student leaving the School. The possible reasons for LOA are as follows: 1. Medical: Emergency or unavoidable medical treatment for student. A licensed doctor’s note with the doctor’s signature must be provided to the school. 2. Personal: A personal or family emergency or other special circumstances that make attendance impossible. 3. Military: Reserve or active duty. PROPER DOCUMENTATION IS REQUIRED FOR ALL LEAVES OF ABSENCE. F1 International students who take a non-medical leave of absence are not allowed to remain in the U.S and must return to their country of residence, otherwise approved by School in extenuating circumstances. Any leaves of absence from school cannot be abused to extend the student’s transfer period between schools. The school reserves the right to decline the student’s leave of absence for a vacation for any reason. Students should follow the steps below if they need to take a leave of absence: 1. Talk to the Office Manager and fill in Leave of Absence Request Form (LOA). 2. The school requires students to fill in the LOA Form, provide the reasons for leave in writing, specify the leave and returning dates, and notify the school staff at least one week before actual absence, unless there is an emergency situation. 3. The School Director must sign the form. 4. After the appropriate administrator signs the LOA Form, then the student can start the leave as of the date listed on the request. 5. Keep in contact with Amlotus during the Leave of Absence (LOA) Period. 6. The student should contact the school one week prior to returning from an LOA. Upon return, an administrator will speak with the instructor who will conduct an evaluation to decide if the student can follow the on-going course. If the instructor decides that the student has fallen too far behind to join the class in session, the instructor will inform the Office Manager. The Office Manager will arrange to place the student in the next available course or make-up classes. 7. If the returning date passes, and the student has not notified Amlotus within 14 calendar days, the student will be considered dropped from the course. AMLOTUS, LLC STUDENT HANDBOOK PAGE 21

8. If the leave extends beyond thirty (30) days, the student will be terminated from the school. If and when the student returns, he/she should make an appointment with the administration in order to apply for re-admission. 9. Please see “Class Make-up Policy” related to LOA for missed class work/ make-up procedures. 10. Tuition Related Policies: No refund will be issued for classes missed while student was on LOA. e. Authorized Withdrawal from Course (AW) A student may withdraw from a course if he/she undergoes impractical circumstance in which the student is unable to complete the course. Any withdrawal must be approved by School Director PRIOR to the student taking action. Students shall follow the steps below for the withdrawal process: 1. Talk to the Office manager and Fill in a Withdrawal Request Form (WR) in advance. The school requires students to fill in the Withdrawal Request Form (WR), provide the reasons for withdrawal, sign the form, and formally notify an Office manager of the withdrawal request. 2. The Office Manager, and the Academic Coordinator will complete the Withdrawal Form and submit it to the Academic Director. An Office manager, the Academic Coordinator, and the Academic Director will sign the Withdrawal Form which means the school received and approved the request. The signed Withdrawal Form will be kept in the student’s folder. 3. Consequences of Withdrawal. Withdrawal from a course/ program means that a student did not complete his/her studies. Instead of receiving a normal grade, he/she may receive an “AW” (Authorized Withdrawal) grade. The student will not be able to graduate from Amlotus with a certificate. A grade of AW does NOT affect a student’s overall cumulative grade. 4. Tuition Related Policies. If a student withdraws and requests a tuition refund, the student needs to fill in the Refund Request Form (RR). Based on the School Refund Policy and the student’s last date of physical attendance in the class, the school staff will calculate the appropriate amount of refund to the student. After the Refund Request Form is approved by the President, the refund check will be ready within forty-five (45) days. “The failure of a student to notify the director in writing of withdrawal may delay refund of tuition due pursuant to Section 5002 of the Education Law.\" f. Minimum Passing Grade: C- (70%) All students must maintain a GPA of 70% (C-) or higher in all courses at all times, and will be periodically evaluated by instructors to determine satisfactory progress. Students failing to maintain a GPA of 70% (C-) may be placed on probation and will be required to repeat the course. Tutoring and remediation services are available for any student needing help. (Please see Academic Probation below). ★ Please note that attendance, homework, efforts and other student engagement behaviors such as class participation will NOT be a part of the grade. AmLotus recognizes that attendance and participation are critical for any learner’s success. However, our goal is for learners to master the student learning outcomes of each of the courses/modules we offer. Therefore, academic progress is determined by direct assessments that measure these rather than attendance, homework, and participation. h. Student Evaluation Students receive midterm and final grade reports. If the student is not progressing satisfactorily at the midpoint, he/she will be informed by an instructor of any problems or deficiencies, and given a chance to rectify these problems, thus avoiding probation. i. Academic Probation Academic probation is an indicator that the student is not fulfilling the requirements of the curriculum. 1. An action for academic probation will take place if: a. The student’s final GPA for the course is below 1.7 (70%) – a letter grade of “C-.” 2. The determination of probation is made by the Instructor and Academic Director of the school to place a student on academic probation for one marking period if the student would otherwise be dismissed pursuant to the requirements set forth in paragraph (1). 3. The school shall dismiss any student who at the end of such academic probation fails to raise his or her cumulative average to 1.7 (70%). Actions taken upon academic probation: a. The instructor and the Academic Director will have a meeting with the student, informing the student in writing that he/she will be on academic probation and completing a student counseling form that includes possible solutions, make-up projects, and tutoring schedules. The director and the student will both sign the student counseling form. The academic probation letter will be placed in a student’s file. b. The probation will last one quarter. During that time, the student must check in with the academic director at least once per week and the academic director will inquire about the academic progress from both the instructor and the student. c. The student and the Academic Director will have a formal meeting at the end of the marking period. If the student’s GPA is over 1.7 (70%) which is the requirement for the satisfactory progress, he/she will be removed from Academic Probation and stated on the following Progress Report/Record. If not, the student will be terminated. The counseling form completed will be placed in the student’s file. d. The student who fails to maintain the satisfactory academic progress during the academic probation is prevented to retake a course and dismissed immediately at the end of the marking period. e. j. Repeating Levels or Courses Students must maintain normal academic progress. Students are to repeat a level only under the following circumstances: 1. Students must repeat the level if they fail to meet satisfactory academic progress (below 70% - a grade of C- in meeting SLOs) at the end of any marking period. In this case, repeating the same course is only allowed one time. 2. Students moving from General English (Level 5) to an elective course (Level 5), iBT TOEFL (Level 5), or Intensive Business English (Level 5) 3. Students moving from an elective course to another elective course (Level 5), iBT TOEFL (Level 5), or Intensive Business English (Level 5) 4. Students moving from iBT TOEFL (Level 5) to an elective course (Level 5), or Intensive Business English Track (Level 5) 5. Students moving from Academic Track, IBE or Master TOEFL at Level 6 to different Tracks at Level 6 Students moving from any track at Level 5 into the Intensive Business English Track must enter Intensive Business English at Level 5 to acquire foundation contextual language and knowledge before moving up to IBE at the final level. Students must show academic progress and are not allowed to take any level down course. k. Graduation Requirements In order to fulfill the requirements for graduation, a student must: 1. Have obtained a minimum grade of “C-” (70%) in each course; 2. Have fulfilled all financial obligations with Amlotus. l. Certificate of Graduation At the end of each course, a Certificate of Completion will be granted to students who complete the course. At the end of the program, a Certificate of Program Completion will be granted to qualified graduates within one week after the completion date. m. Award of Excellent Attendance If a student has attended all of his/her scheduled classes on time for the duration of his/her program, he/she will receive “Award of Excellent Attendance.” The administration and instructors will make a decision of who receives the awards. 8. SCHOOL SCHEDULE AND CALENDAR OF COURSES Amlotus Intensive ESL programs run all year and utilize the academic calendar of quarters. Each session lasts 12 weeks. All new courses are open dependent on student enrollment. Minimum 3 students in enrollment are required to open a new course. Applicants may consult with a school agent to determine the next scheduled starting date. Schedule of Courses · Weekday classes are scheduled in the morning, afternoon, and evening between the hours of 8:00 a.m. and 9:00 p.m. · Saturday & Sunday: Currently weekend classes are not running. School Operation Hours · Monday – Friday: 8:00 a.m. - 9:00 p.m. // Saturday: 9:00 a.m. – 4:00 p.m. · Sunday: School Closed Amlotus currently does not offer the weekend classes. This schedule may be subject to change at school’s discretion. Please contact the school for further information regarding the availability of the weekend classes. 9. STUDENT SERVICES a. Orientation Once students have been admitted, the prospective international students are given a Pre-arrival packet that includes information about academics, lawful immigration procedures and statues, and life in New York through USPS mail or email. On-site Student Orientation for all new students is conducted approximately one week prior to the students’ first day of class. Based on the individual’s academic objectives, the student is offered advice on curriculum and course selection by the Office Manager. The purpose of orientation is to better familiarize students with the school environment and to provide important information necessary for successful course completion. This is also an opportunity for students to ask questions about the school, curriculum, and hand in any final registration documents. Family members are welcome to attend. Digital copies of the Student Handbook are available to all prospective students. Upon request, an admitted student will receive either a digital copy via email, or a printed copy. The Office Manager will review the Handbook with the student and answer any questions the student may have. The student must sign a Handbook Acknowledgement Form acknowledging receipt and comprehension of the information contained in the Student Handbook. b. Consultative Services Amlotus has office managers, directors, school agents, and international student advisors (DSOs) available to answer to the questions regarding school and immigration policies and regulations, along with your personal concerns or matters. They are available from 9:30 am to 6:00 pm on Monday through Friday, and can be reached by telephone at (212) 912-0100, or via email at [email protected]. Emails are checked on a daily basis. Students will also be provided academic advising/testing, and advising on college admissions from teachers during the teacher’s office hours upon the request. AMLOTUS, LLC STUDENT HANDBOOK PAGE 27

c. Student IDs AMLOTUS students will be issued a photo ID upon registration. To request an ID, students should contact the Office Manager as soon as they are registered for classes. Students should carry their IDs with them at all times while in attendance at the school. AMLOTUS student IDs entitle students to any applicable benefits which students receive. Applying for the first student ID card will be at NO cost. If the ID card is lost, each additional ID will cost $10.00. The student ID entitles the student to discounts with any company that offers discounts to students currently enrolled in school. d. Lab Hours The computer lab (Classroom II) is available for individual student use when the room is not being used for class. Students are not permitted to download software onto these computers, but they are free to use the computers for independent and classroom practice. e. Computer & Tutorial Lab (at No Cost): Normally, tutoring is available for 4 hours per week in the Computer Lab with an experienced Tutor to assist students with lessons and studies. Computer Tutorial Lab Schedule*: Tuesdays: 5:30 – 7:30 Fridays: 3:00 – 5:00 * Students please talk to an AMLOTUS Agent for current Computer Tutorial Lab Schedule. f. Student Records Keeping Period Student Enrollment Agreements and payment records shall be kept on site for a maximum of 7 years. After seven years, the records may be moved off-site, but students will still be available for a maximum of 20 years. Students may request records at any time within the given time period. g. Transcripts for Students Within 20 years of the date of his/her graduation, a student may request that Amlotus supply an official (sealed) or non-official transcript. A Transcript Request Form must be completed by the student. There is a $10 charge for each transcript. h. Lost and Found All lost and found items are to be reported to Amlotus Agents, and these items will be kept in the “Lost & Found” box. A student should immediately contact an Agent for information if he/she realizes any item has been lost at the school. After 30 calendar days, the school will not be responsible for keeping these items. AMLOTUS, LLC STUDENT HANDBOOK PAGE 28

i. Housing At the time of this publication, AMLOTUS does not supply student housing. However, students may contact an AMLOTUS Agent to find out about NYC housing information, such as related websites, etc. j. Health Insurance Amlotus does not provide students with any type of health insurance. We strongly recommend that each student obtain an individual insurance policy in case of illness or emergency. There are serious consequences that can be associated with lack of health insurance. In the United States, medical care is not free, and the U.S government does not offer it at a low cost. Therefore, in any emergency or catastrophic care, medical care services can be very expensive, which can jeopardize a student’s ability to maintain financial obligations and consequently affect a student’s ability to maintain a full-time status, which is required by DHS immigration regulations. Amlotus is not responsible for any students incurred by illness, accident, or emergency medical treatment. During Orientation, each student will be given a referral list which will contain the names and addresses of places where he/she might be able to obtain health insurance. If a student needs additional referrals, the Office Manager will be glad to assist the student with that. k. Field Trips Amlotus schedules field trips for students in each ESL course. The instructor plans these trips, and the students get exposure to different activities. If fees are required, students are informed in advance of the fees required and responsible for any cost related to field trips. Students also should review rules of safety below with their instructor before leaving the campus. Amlotus recognizes the importance of field trips and recreational activities as part of the entire ESL learning experience. The school also recognizes the importance of safety during this time. With that in mind, Amlotus has adopted the following safety procedures that each and every instructor MUST go over before embarking on a field trip or recreational activity. · Classes must meet at Amlotus. The instructor will take attendance and the students will leave the campus together. · All students must sign a waiver of injury form and return to the Office Manager. · All students should bring cell phones with them. · Students should have the school phone number, as well as the instructor’s cell phone number. · Students should make the instructor aware of any health-related issues (food allergies, asthma, etc.) prior to leaving the school. · The instructor should explain to the students clearly the route (buses, trains, etc.) that will be taken to and from the field trip, as well as the actual destination address. · Students should IMMEDIATELY contact their instructor or the school if they get separated from their class. · In cases of extreme emergencies, students should call 911 or contact the nearest police officer. · At the end of the trip, the instructor will again take attendance to make sure that all students have returned to the camps safely. · If ANY problems arise on a field trip, the instructor and the student should contact the Academic Coordinator IMMEDIATELY to inform him or Office Staff of the problem. 10. REGULATIONS FOR MAINTAINING YOUR F-1 STATUS (Applicable for F-1 International Student Only) The United States has immigration laws governing foreign students residing within its borders. It is critical that F-1 international students be aware of and fully comply with the following regulations: 1. When arriving in the United State, F-1 international students must report your arrival to an international student advisor (ISA) of your school indicated on your F-1 visa within 30 days. 2. The student must maintain a valid passport at all times. 3. The student must register full-time (18 hours a week) and complete a full course of study during the academic year. If failing to enroll as a full-time student, your F-1 status will be terminated, with exception if you receive permission from your ISA BEFORE dropping below full-time enrollment. 4. As a F1 student, you must maintain normal academic progress. You must repeat the course/level if you fail to meet satisfactory academic progress in a grade of C- (70%) at the end of any marking period. You cannot take the same course no more than twice including failing a course. If you fail the same (repeating) course, you will be terminated immediately, which will result in a loss of your F1 visa. 5. As a F-1 student, you must maintain an 80% attendance rate for each schedule for the duration of any 12-week period. Failure to comply with it will result in the loss of your F-1 student status. 6. If the students’ I-20s expire before they complete their program, students must apply for an extension before the I-20 expires. Please contact your International Student Advisor (ISA) 30 before the expiration date to file an extension. This will require current financial information. 7. Do not remain in the United States beyond your authorized stay. F-1 students are responsible for making sure that their I-20 does not expire, which includes correct level, current funding and a valid program date. 8. If F-1 students complete their program prior to the expiration date listed on the I-20, they must contact the ISA prior to graduation to shorten their expected program completion date. 9. Students with serious academic difficulties, or a medical condition, may apply for Leave of Absence (LOA). A note from the student’s academic advisor or doctor is required. LOA cannot be extended more than 5 months and will be monitored by U.S. Immigration and Naturalization Service (USCIS), via SEVIS. 10. Under no circumstances students are allowed to work off-campus in the United States. Working outside the school is a violation of status and illegal for international students unless it is Official Practical Training or Curricular Practical Training authorized by the USCIS. Amlotus does not offer OPT or CPT at this point. AMLOTUS, LLC STUDENT HANDBOOK PAGE 30

11. F-1students are permitted to work on-campus up to 20 hours per week when school is in session, and full-time during vacation periods. F-2 dependents are not permitted to work. If students have been approved for authorized employment by USCIS, they must pay income tax. 12. The student must report any changes regarding address, name, email, phone number, program to the ISA of the school within 10 days, along with any changes in F2 dependent information. Then, school will update the changes and notify the USCIS via SEVIS. 13. The student must report any change in their financial documentation/ sponsor to their ISA within 10 days. Your I-20 must be updated if your financial information changes. 14. The student must notify their ISA of any changes in the information of accompanying dependents in F-2 status including biographical, relationship, and immigration information. 15. F-1students who would like to transfer to a new school after graduation or during a language training program must complete the transfer process via SEVIS. Please notify your current International Student Advisor (ISA) of your intention to transfer, obtain an acceptance letter and transfer release form from a new school, and submit them to your current school’s ISA. Then, the current school’s ISA will release your information to a new school 16. At Amlotus, vacations are not permitted while your course is in session. However, students are free to take a 4-week break after a 12-week course. Or you can take an 8- week vacation after two consecutive courses (two semesters: 24-week- no break in-between). 17. If the student plans to leave the U.S during school vacation or recession, he/she must report to the ISA before making travel plans. The ISA will endorse your I-20. Failure to do so may cause your reentry to the U.S. to be complicated at the port of entry. The ISA’s signature is good for one full year. 18. If you withdraw or are dismissed, you must inform your ISA immediately. If you do so in advance, you have 15 days to leave the U.S. If you do not inform the ISA, your status ends immediately. If you complete your program of study, you are entitled to have 60 days grace period in which you can remain in the U.S up to the end of the grace period. Once completing your program of study, you must depart the U.S in a timely manner. Failure to do so will result in ‘unlawful presence’, which will prevent you from re-entry to the U.S for the three or ten years. 19. Any time you have a question regarding your immigration status, please ask your ISA for legal immigration advice. 20. It is the student’s responsibility to maintain his/her status while studying at Amlotus. The ISA at Amlotus will assist you in every possible way, but ultimately you are responsible for your own actions. IMPORTANT NOTE: This information is subject to change without notice. 11. REFUND POLICY & PROCEDURES Quarter Ø 12 weeks A. A student who cancels within 7 days of signing the enrollment agreement receives all monies returned with the exception of the non-refundable registration fee. B. Thereafter, a student will be liable for 1. the non-refundable registration fee plus 2. the cost of any textbook or supplies accepted plus 3. tuition liability as of the student’s last date of physical attendance. Tuition liability is divided by the number of quarters in the program. Total tuition liability is limited to the quarter during which the student withdrew or was terminated and any previous quarters completed. (a) First Quarter School may keep If termination occurs 0% Prior to or during the first week 25% During the second week 50% During the third week 75% During the fourth week 100% After the fourth week (b) Subsequent Quarters School may keep If termination occurs 25% During the first week 50% During the second week 75% During the third week 100% After the third week C. Tuition Refund Process 1. If a registered student wishes to cancel, he/she must do so in writing, submitting a Course Withdrawal Form and Refund Request Form, to Amlotus Admission Department, 500 8th Ave Suite 909, New York, NY 10018 or by email to [email protected]. Students can also complete the forms and submit them to their school agent. 2. Then the refund calculation form is completed by a school to determine refunds owned to students who cease instruction. 3. Any refund to a student shall be paid by the school within forty-five days of the date on which the student withdraws from the program. Such date shall be the earliest of (i) the date on which the student gives written notice to the school Or (ii) last date of physical Attendance. 4. If a student has failed to attend classes for a period of fourteen (14) calendar days, the school shall send by regular mail a notice to the student that the student shall be deemed to have withdrawn from the program if the student does not notify the school to the contrary within twelve days from the date on which the letter is sent. 5. If the student fails to respond within such twelve-day period, the student shall be deemed to have withdrawn pursuant to Section 5002 of the Education Law. The failure of a student to notify the school director in writing of withdrawal may delay refund of tuition. 12. TUITION AND REIMBURSEMENT INFORMATION The Tuition Reimbursement Fund has been set up to protect the financial interest of students who attend a New York State licensed proprietary school. If a school closes due to certain reasons while a student is in attendance before the completion of instructional program, the student may be eligible for a refund of paid tuition expenses. If a student drops out of school before completion and he/she files a complaint against the school, and the State Education Department finds any factual support that the student’s complain is valid and determines that there is a violation of Education Law or the Commission’s Regulation as specified in Section 126.7 of the commissioner’s Regulations, the student may be eligible for a tuition refund. In order to file a claim to the Tuition Reimbursement Fund, a student needs to file a complaint with the Bureau of Proprietary School Supervision, New York State Education Department, located at 116 West 32nd Street, 5th Floor, New York, NY 10001. The staff of the NYS Education Department will assist the student regarding the preparation of the Tuition Reimbursement Form. 13. HEALTH AND SAFETY AMLOTUS, LLC STUDENT HANDBOOK PAGE 33

a. Alcohol / Smoking / Drug-Free Policy Amlotus is an alcohol-free school. Any student found in violation of this policy is subject to dismissal. There is no smoking anywhere in Amlotus classrooms or offices. Amlotus is subject to all local, State, and Federal laws related to the possession, use, distribution, manufacture, or sale of drugs or other illegal substances. No one may use, possess, sell, or distribute drugs or other illegal substances or paraphernalia on school property or any school-sponsored activity. This policy also applies to the abuse of legally acquired prescription drugs and medications. Any student apprehended with drugs and/or paraphernalia is subject to dismissal and risks prosecution. b. Campus Security Amlotus makes every effort to provide a safe environment for its students. Our building has a Security Officer at the front desk in the lobby, and anyone not known by that officer must sign in in order to be allowed admittance. If a crime is committed, a police report must be completed and filed. Students are required to report any and all occurrences of a crime to Amlotus personnel. If a sexual assault occurs on campus, the victim and witnesses to the crime must report the incident to the school administration. The police will be contacted. c. Sexual Harassment Sexual harassment is defined as unwelcome sexual advances, requests for sexual favors, and other verbal or physical conduct of a sexual nature that tends to create a hostile or offensive work environment. Any student or employee of Amlotus who is a victim of a sexual offense in the facility or a school-related activity must report the incidence at once to the Director. An investigation will take place. d. Emergency & Evacuation Procedures for Students To All Students: If an EMERGENCY should arise, such as FIRE or SMOKE, or if you hear an alarm from the building, please do as follows: 1. Stay calm. 2. If you SEE SMOKE, but don’t hear an alarm, quickly go to the hallway and make a left. Wait for instructions from Amlotus Administration or building staff. AMLOTUS, LLC STUDENT HANDBOOK PAGE 34

3. If you SEE A FIRE, immediately PULL THE RED PULL STATION (next to the Red Emergency Fire Phone Box) to inform the local Fire Department. **You are at: 500 8th Avenue Room 909, between 35th and 36th Streets, closer to 36th Street. Call 911 on your cell phone or ask someone to call 911 immediately. 4. Follow School Faculty and Staff’s Instructions. 5. Leave School As Soon as Possible. 6. Do Not Use Elevators! Use Exit & Stairs to Go Down to Street. 7. Don’t Open a Door if it is HOT! Before opening a door if you suspect a fire, use the back of one hand to touch the door gently to see if it is hot. If it is hot, do not open the door! 8. If You Become Trapped in a Room During a Fire: - Stay calm and try to take steps to protect yourself. - Put Wet Paper Towel or Wet Cloth over your nose & mouth. - If there is smoke, bend down as close to the floor as possible. - If possible, move to a room with an outside window. - Do this even if you can see Fire Department Personnel from the window. - Stay where Rescuers can see you through the window and wave a bright-colored item to attract their attention. 9. Once you get to the outside, move clear of the door, allowing others to exit. 10. Evacuate to a distance of At Least 500 Feet from the building and stay out of the way of Emergency personnel. Do not return to school unless instructed by School Management. 14. STUDENT RIGHTS The Family Educational Rights and Privacy Act (FERPA) offers students certain rights with respect to their educational records. AMLOTUS, LLC STUDENT HANDBOOK PAGE 35

1. Students enrolled at Amlotus shall have the right to inspect and review the contents of their education records, within 45 days of the day the institution receives the request for access. Students may request to review their education records by submitting a written request identifying the record(s) the student wishes to review to the Office Manager or School Agent. The institution will arrange for access and notify the student of the time and place where the records may be inspected. 2. Any personal information regarding students (including attendance and grades) will NOT be released to any third parties without expressed written consent of the student. 3. A student’s education records are defined as files, materials, or documents, including those in electronic format, that contain information directly related to the student and are maintained by the institution, except as provided by law. Access to a student’s education records is afforded to school officials who have a legitimate educational interest in the records. A school official is defined as a person employed or engaged by the institution in an administrative, supervisory, academic or support staff position; a person or company (including its employees) with whom the school has contracted (such as an attorney, auditor, consultant or collection agent); a trustee serving on a governing board; or a person assisting another school official in performing his or her tasks. A school official has a legitimate educational interest if the official needs to review an education record to fulfill his or her professional responsibility or commitment to the school or conform with the law. 4. Students may request that the institution amend any of their education records, if they believe the record contains information that is inaccurate, misleading or in violation of their privacy rights. The request for change must be made in writing and delivered to the President, and must identify the part of the record the student wants changed and the reason for the requested change stated fully. 5. The written consent of the student is required before personally identifiable information from education records of that student may be released to a third party, unless the disclosure is otherwise allowed under an express FERPA exception to disclosure or is required by law. 6. A student who believes that the Amlotus has violated his or her rights concerning the release of or access to his or her records has the right to file a complaint with the U.S. Department of Education. COMPLAINT PROCEDURES Amlotus encourages all students to openly discuss any questions or complaints they may have related to school’s academic affairs and management with the staff of Amlotus. 1. If a student has a complaint, he/she should first inform the office manager, an agent or instructor. The agent or instructor shall discuss the problem with the student and try to find an appropriate way to resolve it. If the issue is with an instructor, the student should first go to the instructor in an attempt to resolve the issue. If the issue is with school operations or policy, the student should go the office manager. If the problem is not resolved, the agent (or student) can report the problem to the School Director by submitting a student complaint form. 2. The School Director will arrange a meeting with the student and any related Amlotus staff and try to resolve the problem. The meeting will be documented by the School Director, and both parties will be given a chance to present any facts or proof relative to the complaint. The School Director will attempt to bring resolution to the complaint. In case the student is still not satisfied with the solution, he/she should contact the President in writing. 3. The President will schedule a meeting with the student, staff members, and School Director in an attempt to solve the students’ complaint. The President will document the meeting, including the outcome and resolution, or lack of. 4. If the student still is not satisfied, he/she has the right to send the complaint in writing to New York State Education Department. DISCLOSURE STATEMENT AMLOTUS IS LICENSED BY THE NEW YORK STATE EDUCATION DEPARTMENT. ALL PROSPECTIVE STUDENTS SHOULD READ THE ENTIRE HANDBOOK FOR FACTUAL INFORMATION BEFORE REGISTRATION. THE STUDENT HANDBOOK INFORMATION MAY BE SUBJECT TO CHANGE.
