Wow, what a year all because of YOU! We have seen significant growth throughout the past two years since our incorporation as a non-profit. YOU are one of 700 donors that joined the Horizon Health Foundation. HORIZON NURSE SAVES TWO LIVES...GAINS A FAMILY Sharon Egleston, a nurse at Horizon’s Mission Community Health Center has touched many lives over the course of her career. However, one of those lives recently brought her the most rewarding experience she could’ve dreamed of – a new daughter, Zoe. One day, the clinic received a call from the local shelter stating that they had a young woman who was pregnant and in need of OB care, but had no means of payment. Sharon asked for her to come in and take a pregnancy test, so she could apply for Medicaid, which she agreed to. Personally, Sharon and her husband have struggled with infertility and miscarriages for years and had begun to research adoption. As the young woman was reviewing her own options and looking for the best possible future for her unborn child, a worker at the shelter informed her that the nurse, who had been caring for her at the clinic, and her husband were unable to have children and may be interested in adoption. During her next visit to the clinic, the young woman approached Sharon and asked if her and her husband would like to adopt her baby. Shocked at the turn of events, Sharon discussed with her husband and they agreed that they wanted to pursue the adoption. Now, months later, the Eglestons are eternally grateful for the beautiful daughter they have been given – only because a patient was able to receive the care she needed through Horizon Health Care and the Horizon Health Foundation. 4

HORIZON EMPLOYEES RAISE NEARLY $125,000 IN 3 YEARS In October 2018, OF HEALTHCARE EMPLOYEES, THEthe Horizon Health Foundation kicked off HEART their third Employee Giving Campaign, “Growing for Good.” Through this campaign, Horizon Health Care employees across the state were challenged to raise $45,000. Employees shattered last year’s total of $43,000 and raised over $48,000. Since launching the Employee Giving Campaign in 2016, total contributions have exceeded over $124,000. All donations raised through the employee campaign will be designated to the Horizon Health Foundation’s funds of behavioral health, dental care, cancer care, charity care, diabetes, employee emergency, service area clinics, and the Horizon Impact – our greatest needs fund. Donations raised will be used to benefit Horizon’s patients and local clinics in these fund areas. The annual Employee Giving Campaign is led by a committee made up of Horizon employees from across the organization. Lacey Finkbeiner of Isabel, SD, is one of the individuals who has been involved since the first campaign. “Being a donor and committee member of the Horizon Health Foundation has been a true enlightening experience since day one,” said Finkbeiner, Clinical Informatics Coordinator. “Knowing the impact of my donation to help a child receive dental care, a woman to receive an annual wellness exam, or a patient offered the means to get them to their specialist appointment helps ease my comfort of mind that Horizon is truly working to keep our communities well. IMPACTING SOUTH DAKOTA FAMILIES...CHARITY CARE In 2017, Horizon Health Care in conjunction with the Horizon Health Foundation created a new Charity Care Program. This program supports patients who struggle due to unforeseen circumstances in their life. For example, a man who was diagnosed with a brain tumor, had several surgeries that left him with enormous medical bills, and then lost his house and belongings in a fire was helped by the Charity Care Program. GIVING SOUTH DAKOTA KIDS A SMILE...GIVING TUESDAY 2018 The days after Thanksgiving are commonly known for shopping deals across the state, but this year it was the Tuesday after Thanksgiving that has left a memorable impact across South Dakota. Horizon Health Foundation, in conjunction with South FOR Dakota Gives, raised over $17,600 on November 27th during South Dakota’s inaugural “Day of Giving” Giving MILES Tuesday event. Delta Dental of South Dakota also partnered with the Foundation to support the event. Horizon Health Foundation, one of nearly 300 participating nonprofits taking part in the event, succeeded their goal of $10,000 during the one-day event. “We are so thankful to all of our friends that took action and made a donation on Giving Tuesday to support children in need of dental care across South Dakota. Each donor and gift will make an impact in a child’s life and we thank you for that,” said Tracy Pardy, Executive Director of the Horizon Health Foundation. All funds raised from the event will benefit Horizon Health Care’s annual “Smiles for Miles” program, which is a month-long promotion in October at all eight Horizon dental clinics, allowing children ages 0-5 to receive a free dental exam, cleaning and fluoride treatment. The Smiles for Miles program encourages early prevention oral health care among children in our communities. Giving Tuesday is a global day of giving that harnesses the collective power of individuals, communities, and organizations to encourage philanthropy and to celebrate generosity. 13

STUDENTS RAISE NEARLY $13,000 FOR PATIENTS In October 2018, high school students from Howard High School and De Smet High School joined forces with the Horizon Health Foundation to raise awareness and nearly $13,000 for patients affected by cancer in their respective counties. All funds were raised through t-shirt sales and went to the Foundation’s Cancer Care fund. HORIZON HEALTH FOUNDATION STAFF TRACY PARDY, Executive Director Background: I grew up in Onida, SD with my loving grandparents who raised me from the age 5. Perhaps this is where I learned the ways of the world, my love for cooking from scratch, and a fondness for the elderly. Adam, Jo, Tracy & Brianna I began my career in Philanthropy 18 years ago working in higher education, and the past 8 years in healthcare. I am incredibly lucky to have this dream career - to meet people all across the state that want to help change lives. I received my Bachelor of Arts Degree in Organizational Management from Ashford University and currently serve as the Vice President for the South Dakota Chapter of the Association of Fundraising Professionals. Family: Husband, Patrick, son, Tucker (6 - Kindergarten), and Stepchildren: Zach (Senior), Kayla (SDSU Pharmacy), Nick and Shelby (farming/ banking) Interests: I enjoy life, it’s a gift! First and foremost, my favorite is spending time with my family and close friends. My passion is food and wine - I enjoy relaxing with a nice cabernet while reading through cookbooks to discover new dishes and then preparing them for my family and friends. I enjoy the beautiful South Dakota outdoors - whether exploring during a hike, at the river or along the beach, or taking in the seasons. Mostly, I believe in channeling my inner childhood by turning into a T-Rex while chasing my son around the house! :-) ADAM EGGERT, Database/Development Assistant Background: I grew up in Howard, SD graduating from Howard High School in 2001. After high school, I attended Southeast Technical Institute for Business/Financial Services. Prior to Horizon, I spent the previous 11 years in the banking/credit union industry in a variety of positions ranging from teller to loan officer. My wife and I recently got married in September 2018 and we live in Howard. Family: Wife, Lexy and cat, Summer Interests: Golf, Golf, and more Golf! I also enjoy spending time with family, and attending or watching any sporting events. BRIANNA FELDHAUS, Fundraising Officer Background: After graduating from Howard High School, I went to the University of South Dakota, earning a Bachelor of Arts in Communications Studies, Criminal Justice, and Spanish; all while working in Residence Life as an RA and with the Medical School as an Administrative Assistant. Then I transitioned to the University of Kansas, receiving a Master’s of Science in Policy and Administration; while again working in Residence Life as a Hall Director and also with Legal Student Services as a Clerk. After graduating I continued to work for university systems within Residence Life until the summer of 2015 when I transitioned to Horizon Health Care as a Recruiter working within the Human Resources Department. Family: Husband, Eric Interests: Enjoying the great outdoors, watching the Food Network Channel, and dreaming up the next ‘Field to Plate’ locally sourced dish. I also enjoy helping around the family farm (cattle and grain). Why did you choose to join the Foundation? The opportunity to have a larger platform to empower and inspire our patients by providing funding that serves our communities that would otherwise not have access to healthcare. I believe everyone should have access to high quality affordable healthcare, and the opportunity to be Horizon Health Foundation’s first Fundraising Officer is very humbling and gratifying! JO RICE, Senior Administrative Assistant Background: I was born and raised in Flandreau, SD and currently reside in Howard, SD. My career has channeled around the health care services both in Flandreau and Howard. I moved to White River, SD for four years and was working at the medical clinic at the time Horizon Health Care, Inc., purchased that facility. This is my 19th year of employment with Horizon. Family: Four children: one daughter, Karla; three sons Kevin, Duane, and David. Ten grandchildren and 4 great- grandchildren. My siblings consist of one brother and two sisters. Interests: I love to read; and like anyone with children, grandchildren and great grandchildren partaking in all activities that comes their way. 21

HORIZON HEALTH FOUNDATION BOARD OF DIRECTORS OFFICERS: The Horizon Health Foundation Board of Directors shapes the Foundation’s direction through its mission, HOWARD development plans, budget, and key policies. Our board ensures that the leadership, resources, and John Mengenhausen finances in place match the Foundation’s vision. The Foundation Board meets formally six times each year, Board Chairman as well as between the regular meetings as needed to conduct the Foundation’s business.
