SuppliesRoutine supplies needed to collect and submit specimens areprovided at no charge to our clients. Supplies are obtained bycalling Customer Service or online via our website. Plan aheadand please allow 3 to 5 business days for delivery of supplies.Professional ConsultationsAntech Diagnostics provides our customers with the mostcomprehensive consulting services available. Our staff of over120 board-certified veterinary specialists can compliment yourexpertise and patient care. Most of our professional servicesare provided at no additional charge to our customers. Formore detailed information about our consultants, please skip tothe Professional Staff/Consultants section tab.PAYMENT TERMSAll fees are charged or billed directly to you. Payment in advance is required for all clients who haven’t established credit withour company, or for clients who have a poor payment history. The stamp contains all of the necessary billing information tocorrectly invoice Antech Diagnostics for your shipment. Simply fill in your name,phone number, and date of shipment on the stamp; peel off the right side of thestamp and affix it to the outside of the clinical pack bag. Retain the left portion ofthe stamp for your records, in the event your shipment requires tracking.The following are important points to note about the Billable Stamp:• FedEx® occasionally redesigns the billable stamp to enhance it usefulness. Consequently, the stamps you receive may differ slightly from the example shown below.• The “ship to” address on the stamps and the example below is not Antech’s physical address. The address listed facilitates FedEx® ability to deliver our packages earlier than normal Priority Overnight packages. U.S. Mail and other carriers will not be able to deliver to the address on the stamps, therefore the address should not be used for anything other than sample submission through FedEx®. Packaging InstructionsIt is important that specimens are shipped in accordance with FedEx® diagnostic specimen shipping guidelines. In order toensure a prompt delivery, adhere to the following packaging instructions when shipping your package:1. Place your specimen tubes and Antech requisition form into a zip-lock bag. Please separate patient samples into individual bags.2. Position the bag along with an absorbent material, such as a paper towel, inside your Test Express shipping box. You may include multiple samples in the same box.3. Close the box and seal the lid with tape.4. Fill in your clinic information and the date on the FedEx Billable Stamp. For shipments picked up on Friday, please be sure to select the Saturday Delivery Box on the billable stamp.5. Remove the top copy of the Billable Stamp and retain for your records.6.
