Fire-Rescue November 2018 Noteworthy Incidents 11/19/18 Smoke Scare – Station 33 responded to the 600 block of Silverthorn Rd. for the smell of smoke inside a structure. A search of the house was conducted and no evidence of a fire was found. 11/19/18 MVA – Station 33 responded to the 1100 block of Gulf Breeze Pkwy. for a motor vehicle accident involving a motorcycle. One patient was transported to the hospital by Escambia County EMS. 11/20/18 MVA – Station 33 responded to the 1800 block of Gulf Breeze Pkwy. for a motor vehicle accident involving a bicycle. One patient was transported to the hospital by LifeGuard EMS. 11/21/18 Hazardous Condition – Station 33 responded to a natural gas leak in the 200 block of Poinciana Dr. The leak involved a 2” main and Engine 33 evacuated several surrounding residences and stood-by with a charged attack line until the leak was secured. 11/29/18 Smoke Scare – Station 33 responded to the 400 block of Navarre St. for smoke inside a structure. A search of the residence found revealed a burnt-out HVAC blower motor. 07/17/18 Assist to PublicPSaegrev4ic9es – Engine 33 responded to a water main break on McAbee Court.

Fire-Rescue November 2018 Squad 33 Minor coolant leak. Two shops have been unable to find the leak. Members are keeping an eye on it and adding Humvee 33 coolant as needed. Support 33 Out of service with a possible transmission problem. None Page 51

Public Services - Water & Sewer (SSRUS) November 2018 Monthly Highlights: WASTEWATER TREATMENT FACILITY Excessive rainfall in November required Staff to activate A total of all spray fields and West Course irrigation. A portable 12.2 inches of pump was deployed at the stormawater pond behind rainfall was recorded in the month of November. the Sonic Drive-In in order to convey excess stormwater out of the driving range areas and into the West Course Drainage system. Staff pulled the pressure maintainer pump at the East course pump house ordered its replacement. The #1 master lift station pump needs a new impeller and possibly a new volute. The breaker trips due to excessive rags building up because the impeller is so worn down. A bypass pump was installed to assist the master lift station. WATER DISTRIBUTION & SEWER COLLECTION There were no water main breaks in November. The Tiger Point and Ceylon Lift station went into high level due to a valve being turned off by mistake. The area was cleaned and FDEP was notified. Staff identified and repaired several sources of inflow and infiltration (I&I) in the collection system through smoke testing and camera inspections. These activities are high priority for this year to reduce I&I into the collection system by at least 25%. Page 53

Public Services - Water & Sewer (SSRUS) November 2018 OPERATOR CERTIFICATIONS Service Worker I, Bryan Hix is working on the Water Distribution course work. The following Service Worker I's, Eric Dominguez, Cody Foster, Dillon Tyree, Will Berkley, Alan Durnil, and Charlie Laveck, all completed their course work for Water Distribution and are waiting to take the State test. Senior Service Worker, Brad Elliot, is working on his Water Plant Operator Certification course. Construction Foreman, Jeremy Norris, and Service Worker I, Steven Mowery, completed their Water Plant Operator Certification course work and are waiting to take the State test. Public Services - Water & Sewer (SSRUS) November 2018 Strategic Priorities Update: 1. OPERATOR TRAINING PROGRAM Staff will be completing a revised Operator Training Program to develop and retain the required certifications for operating the water, collection and treatment systems. The plan will include revised job descriptions to reflect relevant certification requirements for the various tasks in the field and regulatory requirements, and provide the pathway for employees to progress from Service Worker Trainee to Service Worker I, II, Senior and Foreman. 2. RECLAIMED IRRIGATION SUPPLY PLAN Staff is developing a reclaimed master plan of future reclaimed irrigation in conjunction with potential new developments. Completion expected in March of 2019. Public Services - Streets & Stormwater November 2018 Monthly Highlights: Staff and the inmate crew completed the routine activities of drainage and pump station inspections, roadway and right of way maintenance. Inmates are continuing to cut the City right of ways along with tree trimming and roadway asphalt repairs. Staff extracted and removed sidewalk along South Sunset Avenue and Shoreline Drive. Seven potholes were repaired in various locations throughout the City. Statistics/Performance Data: Page 58

Public Services - Streets & Stormwater November 2018 Strategic Priorities Update: 1. SIGNAGE EVALUATION/REPLACEMENT PROGRAM Staff developed a program for the evaluation and replacement of the street signage throughout the City. Implementation began in 2015, with an initial inspection and evaluation of each sign in the City, and then separating the City into 5 sections. A total of 709 sign locations were identified and those that did not meet the requirements outlined in the Manual for Uniform Traffic Control Devices (MUTCD) were repaired or replaced. City Staff personnel conduct annual inspections, focusing on one section per year. Signs that do not meet the MUTCD standards are repaired or replaced. When City Staff personnel identify signs in the field in the normal course of their workday that are out of compliance, a work order is created to address the issue. In addition, customers call in from time to time, reporting missing or broken signs, and City Staff personnel conduct reparations or replacements accordingly. A total of 297 signs have been replaced since the plan began. City Staff personnel completed the annual inspection on Section 1 in 2017, and completed Section 2 in 2018. 2. STORMWATER SYSTEM VIDEO INSPECTION Staff is developing a program to inspect, with a video record, all stormwater pipes in the City, every 5 years. The program will go out for bid in late 2018, with work to begin in FY 2019. Page 59

Public Services - Natural Gas November 2018 Monthly Highlights: Routine monthly reports of charts stations and odorant reports were completed. Staff continues work on leak survey, pressure checks, and valve maintenance. Staff had an increase in gas leak calls and gas light ups due to the seasonal temperatures dropping. Public Services - Natural Gas November 2018 Strategic Priorities Update 1. SUPPLY MASTER PLAN Staff will review current purchase agreements and suppliers as well as develop any opportunity to purchase all or a percentage of gas supply at a lower cost (Henry Hub versus WCOG). The first formal draft was produced in April 2018, however, this will be reviewed annually after that point. Community Services November 2018 BOARD OF ADJUSTMENT (BOA) Staff received one application for the December meeting; however, the meeting was scheduled for the 26th and the Chairman elected to cancel the meeting and hear the case at the January 2019 meeting. The applicant wanted to make additional changes to the plans and agreed with rescheduling. ARCHITECTURAL REVIEW BOARD (ARB) There were no cases submitted to the ARB for November. CRA ACTIVITY UNDERGROUNDING UTILITIES The Neighborhood Services Coordinator worked with Dewberry Engineers to start finalizing the underground utility study. DANIEL / JOACHIM STREETSCAPING PROJECT The Neighborhood Services Coordinator worked with the Design-Build team on Change Order 1. WAYFINDING SIGNS Ongoing FAIRPOINT GATEWAY Staff met with Gulf Breeze United Methodist Church regarding the City issuing a grant for a special landscaping project. Page 68

Community Services November 2018 SPECIAL PROJECTS 98/399 LANDSCAPING PROJECT Staff worked with the Finance Department to reconcile all the expenses for the Mix-Nutt project for the City Audit. MULTI-USE PATH Ongoing. HIGH SCHOOL GATE The Neighborhood Services Coordinator developed a concept design and received quotes for a gate at Gulf Breeze High School on Daniel Drive. Met with GBHS for feedback Met with 3 fencing companies SOUNDVIEW TRAIL NEIGHBORHOOD ENHANCEMENT PLAN Staff met with the City manager regarding the development of the Soundview Trail Neighborhood Enhancement Plan. MISCELLANEOUS Staff participated in the Traffic Safety and Calming work group meeting. Staff met with Parks and Recreation and Randy Hepworth regarding a special landscaping project at the intersection of 98 & 399. Staff assisted with Gulf Breeze Elementary School’s Annual Bike Fest.
